Because 99% of the time, the person did change something, and just didn't realize or forget they did something. Most people just want to blame the game, when they are the problem. Not a single one of the questions was stupid, no matter how experienced you are does not mean you couldn't make a mistake.
It can still take out groups of mobs in Wildspace while stealthed. It does a bit less damage now since you can't get all those Mythallar set and Oppressive Darkness procs, but quite frankly that was broken to bits. An AoE out damaging ST is stupid.

I believe one of the main reasons people are leaving is because they do not think they will succeed. That is why it is more likely to happen in some dungeons, trials, or skirmishes, than in others. Increasing the reward won't get people to stay, since they see it as they will get no reward.

Did you complete the campaign, or did you get just get the currency for the milestones? You have to do the yellow quests offered by the quest givers to progress the story to get the new quests.

I would guess two possible reasons: 1: what autumnwitch said above, you got the campaign currency to finish it out, but didn't complete the progression (yellow) quests. 2: you haven't finished all 6 milestones. To complete a milestone, pretty sure you have to claim the reward, which you have not done.
