Wait a sec.... so now you NEED three runes in order for a wraith to show? Well that explains why all weekend me and my alliance friends were to get a wraith though we were getting essence drops from the end chests. Does it matter which 3 runes?

I noticed it over the weekend too. I got an alt to 80 and through Undermountain campaign on Sunday, and saw companions left over over the place in the various zones. I thought that they finally got rid of all the immolation spirits floating around, now the orphaned companion bug is back.

I have seen this once too. It was a purple worm boss, so it burrows under the floor and then rises up. Got it on the map where its a platform, the gate is at the opposite end of the entry and the two other side are cliffs.
