Im getting random removals myself, I never know what map will remove my stuff. Mainly space to space, like after grinding my face off at argala, i'll transwarp to k7, idle in sherman for a bit, do doff missions, transwarp back. Go into argala, and I have 2 active space doffs left, and it unequips my batteries.
This continues to worsen on aesculapius@contactpsi
Anytime I log out and back in, transition to a planet, starship bridge, sometimes another space zone, over half of every action bar row loses skills. They're blank.
I have to drag 5-10 skills back to the bar after every transition or logout. I've changed ships, it's now happening on every ship I have.
I've used the loadout system, which started this madness.

WHen I go from space to ground and back to space it resets everyhting. If I make any change to a character abilities on a character unaffected it becomes and issue for him as well. This should have been fixed by now and the fact it is being stalled on patching is a slap in the face of the customers.
As others have said this is not solely a loadout problem, the problem has been present since around season 6 along with other bugs that are still on the go since then, well before loadouts. Loadouts is just a buggy system put on top of the underlying problem.
The game cannot save items in the correct locations properly, & the loadout system can't either. Put them together & we have our current situation. Devs - This is what happens when bugs aren't fixed and new systems & expansions are released on top of them.
It is currently unknown what causes the issue to first trigger for a character or ship.
It'd be nice if we were able to delete a created loadout instead of just saving over it.
I managed to get my T5-U Defiant to be mostly alright after the command tray being completely 100% broken on all 10 rows. After i set it up the way I wanted, I saved the layout to a loadout slot & logged off. When i logged back on the only thing that keeps moving about is the weapon batteries, which is the buggiest of them all in my experience, and Mask Energy Signature from the adapted maco space set. Why logging off makes any difference is anyones guess as it should have no bearing on a tray setup in any way.
I'd love to know if any dev is actually still looking at/reading this topic. I think we, the players, have made far more headway and tracking down the potential root sources for this bug.
Someone said they should make all these settings saved locally, rather than as they are right now, and that's sounding like the only potential fix to me for the abundance of errors that all come back to the same thing: the game not being able to remember stuff.
I mean hell, look at the load-up menu. Mine hasn't displayed my bridge crew of my toons properly in ages. The game can't remember basic stuff from the get-go.
Maybe we need to start calling it Star Trek Alzheimer's or something.
This starting happening to me today, tried every fix i have seen on forum & nothing works. I am level 58 have many things in tray but it randomly changes to basic things like evasive/brace for impact/etc, just the few things i had available when i first got in a small shuttle. When putting everything back to normal i have no miracle worker, hyperdrive slipstream or azure icons & have no idea how to get them back. The "pirate distress call" is in my inventory but wont load back onto tray at all.
Wasted a lot of time putting back what i could (where are my missing things?) but it just screws up again, i don`t even have to leave ship or move in space, just clicking on my inventory caused tray to change the last time.
Hope this problem is fixed soon.
I first noticed the issue a couple of days ago on my account Paul@MustangPauli. Initially the issue was limited to only one of my ships and a few random items in my tray. Since then the problem has spread to all of my ships and now includes other aspects of the profile such as duty officers. The issue crops up whenever I go from a Starbase, bridge mode or ground mode back to sector space. It also occurs whenever I log out and log back in.
And again...... ALL my characters under @richarddover (I've got 17, not going to list them all, deal).
When? Maptrasfer from the Academy to space, or from ESD to space, or from Sol to Sector, or from Sirius to Regulus, or from Sirius to Alpha Centauri, or from Alpha Centauri to Psi Velorum, or from Psi Velorum to Tau Dewa (Seeing a freaking pattern here yet? )
What? Boffs removed, weapons removed, consoles removed, devices removed, in an endless variation of combinations.
Reloading the loadout helps... sometimes. Sometimes it doesn't, in which case I get to scramble to get SOME level of functionality if I've just entered a Fleet Action, or STF, or any kind of space battle situation. It gets me all kinds of compliments from team mates if I'm PUGging, and can't do **** for the first few minutes.
Under no circumstances can my character use "hunker down" which is the 3 piece set bonus for the dyson ground set. I cannot activate the power. It is not in my power tray and is not on a list of buffs/powers when I click the "View all available abilities" button. I have removed it from my character and placed all three pieces on a boff and placed another set on my character. After putting the 3 piece dyson ground set back on, I have the same issue. No 3 piece set bonus. My alt. Surius can use the buff with the 3 piece set. But not my main, Salazar.
When you see "TRIBBLE" in my posts, it's because I manually typed "TRIBBLE" and censored myself.
Happends !!ALWAYS!! on character Aucius@karriq. on ALL of his ships. When i change location. there is two ways. 1)Some of skills dissapearing 2)Skillbar becomes a mess of random skills.
P\S This makes this character completely unplayable.
P\P\S Just fix it already...Its easy. Just let user to store data of his skillbars on his pc.
I mean hell, look at the load-up menu. Mine hasn't displayed my bridge crew of my toons properly in ages.
I wondered about this recently too. After a brief investigation, I realized that the game doesn't show my preset away team in the order in which I set them. It just shows by length in which they have served starting 2nd from the left, 2nd from the right, 1st left, 1st right. This is true for all my toons.

Now i have lost everything from tray, can only reinsert starship traits, scorpion fighters & Jem'Hadar set item, lost all my other skills/powers (did have two rows full). I suppose i should count myself lucky as i was going to take out lifetime subscription tomorrow when my wages got paid, this experience has put me right off that idea. Its impossible to play at level 58 without tray skills so what am i supposed to do now? start again with the worry it will happen to next toon?
Dear Devs, I'm thinking you may need a new perspective. Have you tried searching here for the root of this problem: https://c1.staticflickr.com/3/2103/2251155626_63d7c82709.jpg
Then again, I have a feeling that's where your head is most of the time, so I doubt it will help.
There's obviously a bunch of you still having issues with loadouts. Powers, Boffs and Doffs get changed or unslotted from the reports I'm seeing. We need your help to track this down.
We need characters that consistently experience this issue. Not "most of the time" or "sometimes", but "all of the time" or as close thereto as possible.
If you have such a character, please provide the character's name@handle, and a clear description of what exactly happens with the character, and under what circumstances this is seen.
For example: Enter the Sol System map, switch to <Ship Name>, set the loadout to <Loadout Name>, note the locations of the following power icons <list powers>, and now <action that causes the bug> and note that the previous power icons are now moved.
So, if you've got a character that consistently experiences these loadout issues, and are willing and able to take the time to provide a clear description of how to reproduce the bug with your character, please reply.
I understand this issue is extremely frustrating, and you may want to vent about it, but I'm asking everyone to please limit replies to this thread to the requested information only.
Thank you.

Moonwind Tsukiyomi@Moonwind experiences this glitch every time he is on, I log in, clear my tray, set up my tray, switch instances ex: warp from space esd to mirror invasion and instantly everything is gone and I have to do it again. same happens if I go to a ground map, or undergo any map transfer. My loadout is saved, even reloading it doesn't change anything. it doesn't load, and the random time it does load as soon as I transfer maps it's gone again.

For me, there is no rhyme or reason for how it displays. On my Romulan, it displays 4 copies of my Klingon Faction exchange BOFF, who happens to be a scantily clad Orion lady, so I don't mind that glitch. :P But it's random otherwise.
This may or may not be of any use....
but my loadouts went ballistic when I configured my shuttle, using the same items that were on my big ship. When I did that, every time I changed zones (including entry to STF or missions) it removed all my gear, as if the shuttle were prioritized and the big ship secondary.
The workarounds that many mention here finally reset something and it started working again --- I forget what fixed it but save, remove items, reinstall items, zone, remove, reinstall, zone, load loadout, zone, is it working now?? Yes, something in there fixed it.
Not very much info but that was my experience. I have also had some oddities with
dorf@noroblad who upgraded his JHDC and since then it simply will not keep my plasma leech slotted -- every time I zone, it pops back to my bag. This is still ongoing. I did not spend much time on it and its *possible* his loadout is for the -1 console old version and that is behind it. I need to re-save the loadout and mess with it more.
Firstly, I've gotten this bug with all of my characters at some point or another. If I replace ANY skill that is automatically added to the tray with any other skill, it gets replaced the next time I change maps. I've tried the tray lock, but no such luck. With one of my characters, I've had some luck in the following way:
Veorex@st0rmwyvrn (Alien male) flying a Phantom Intel Escort
For some reason, the bug in this account seems to leave me alone when I put the powers I want in open slots. However, the moment I place any power in any slot with a default set power I can just about guarantee a switch around of powers as soon as I change maps. I learned pretty quickly early one that I couldn't put powers in slots with powers already there, but it didn't really become a serious problem until I hit level 50.
Ki'is@st0rmwyvrn (Romulan female) flying a C-storm Ar'Kif Tactical Carrier Warbird Retrofit, Gyridex allied with Federation:
I've not been able to do anything at all to fix this bug with this account. I can't seem to predict when the problem will occur. Usually, any change of map seems to completely re-write the tray back to default, though sometimes staying in space will keep a few of the items in the tray. Also, sometimes placing powers in open slots in the tray only will help. I just logged in to check the account for info and found that all of the tray items are where I left them last. Keeping in mind that as with my federation character, I just placed powers in open slots last time. However, I've seen the bar completely reset to default after shifting maps even with placement of powers in open slots only. This account has typically reset after any change of map either space->space, or space->ground.
When I reached the level 50 number of powers, I had to rearrange the HUD and start using the second power tray. I've had this experience with both power trays consistently since I started playing the game about the time of Delta release. I basically don't touch my ground toolbar, so I can't really comment on that. Also, while I have a few other characters, I haven't put as much time or energy into them as I have these two, so I can't really give feedback. I can say that I anticipate similar problems with them, thus using the second power tray (which doesn't have as many powers placed in it by default and therefore has more slots that are less likely to reset) should do for me just fine until around level 40-50 or so. Lastly it should be noted that I don't use loadouts.
On a slightly different note, I've been getting an odd glitch on the character selection screen. For the above-mentioned Romulan character, I don't get a preview of the ship but of the legs of one of the characters...? Maybe has something to do with having one of the Reman shields equipped to the ship meaning the skin is slightly off?
Anyway, hope that's helpful in your debugging. I'm very eager to see it get fixed, and I'm grateful for anything you can do!

I'm starting to believe that the problem may stem from the game partially recognizing some small craft as your starship and the loadout issue may be related to this issue:
I have the both this shuttle-name-in-profile bug and loadout bug whenever I activate my Tholian Widow as a small craft. Both bugs disappear when I set other shuttles active.
(if it helps in investigating the loadout bug, the shuttle-name-in-profile bug correctly shows the starship name in the "Your Status" window [default-position, upper-left screen corner], but shows the shuttle name in the "Target Info" window [upper-middle default screen position])
Thank you to those who mentioned shuttle loadout may be connected to this problem. I had recently changed an item on my shuttle before doing that "shuttle race" mission in delta sector & tray TRIBBLE up happened shortly afterwards. I have now removed that item & put tray back in order, so far problem not come back.
After i used shuttle the other day i did notice when putting "pirate distress call" between my starship tray & inventory it was loading (countdown) into inventory but not when i put it in device slot .. like it was doing it wrong way round. I`m now sure that my system tray problem had something to do with using shuttle, have no idea why but glad this forum gave me clues to rectify it.
Human Starfleet Officer Tyree@CaptCanaj
I've been using a single loadout, and trying to add or change my ship configuration on the fly.
I do all my configuration in Sol System, or in STF missions.
I don't save the loadout, I just use one loadout and it's constantly changing. I'm always trying new things.
I should not have to save any loadouts since I only use one: the stuff I put in the tray and on my ship.
So far, swapping loadouts has only caused all my work to instantly disappear, and I have to scramble to find my kit in my inventory and put it back on.
My space skills from a year ago are in the tray, and stay there. When I change their locations, they revert on map change, but that's ok because I like where they're at.
But I can't get new space skills to stay in the tray, i.e. Rock & Roll.
Set skills such as Gravitic Anchor leave on map change.
Consumables such as Shield Battery leave on map change.
Enter STF
New skills are gone from tray
Put them back in and use them
Leave STF
Enter STF
Skills are gone from the tray
put them back in and use them
You get the picture.
Edit
I was just able to save skills in the tray! By configuring them in sector space, not in Sol system of STF.

Under certain conditions, the space and ground trays forget the positions of abilities. This bug is still happening after the patch to Holodeck on November 6, 2014.
"Resolved an issue where items saved to a loadout would occasionally remove itself from its slot when transferring to another map."
Please note that I have never created a loadout on any of my characters. (I'm not sure it even matters for the ground tray.) I have noticed that the system tends to remember the tray configuration from the first time I set up a ship, but forgets any changes made after that. I have fixed several C-Store ships by dismissing and reclaiming them, but not all ships can be reclaimed. In particular, fleet ships, lockbox ships, Lobi ships, and ships claimed using free ship tokens cannot be reclaimed.
Case 1
1. Log on to Aoede@frtoaster.
2. Go to the Qo'noS shipyard.
3. Talk to the ship selection officer.
4. Select the I.K.S. Richard III and then click "Ready Starship".
5. Beam out to the Qo'noS System.
6. Arrange the space tray as shown in the images below. I have drawn colored rectangles to distinguish between different instances of the same ability.
If you cannot right-click drag a bridge officer ability from the tray with the portraits into the space tray, then try unslotting and reslotting the bridge officer. Alternatively, you can right-click drag the ability into another slot and then right-click drag it from that slot into the final slot. However, I am unable to discern why some slots work and others do not. The following abilities and slots tend to be problematic:
B'Ellera's Attack Pattern Omega III into row 2 slot 2
K'Gan's Torpedo: High Yield II into row 2 slot 1
Ch'gren's Emergency Power to Shields II into row 3 slot 8
7. Beam down to Klingon Academy.
8. Beam up to the Qo'noS System. The space tray now appears as shown in the images below.
Notice that certain bridge officer abilities are missing and others have moved into different slots. In particular, the bridge officer abilities noted to be problematic in step 6 are all missing. Furthermore, K'Gan's Tactical Team I appears twice, and the captain ability Tactical Fleet II is missing.
Notes: I had claimed the I.K.S. Richard III before Delta Rising. After Delta Rising, I retrained my bridge officers and rearranged my tray. (I retrained my bridge officers on November 3, 2014, and again on either November 4 or November 5, 2014). Before Delta Rising, I had put K'Gan in the commander station and B'Ellera in the lieutenant commander station; after Delta Rising, I switched B'Ellera to the commander station and K'Gan to the lieutenant commander station. The system seems to be trying to restore the configuration I had before Delta Rising. For example, it puts B'Ellera's lieutenant commander ability (Cannon: Rapid Fire II) into the slot that contained K'Gan's old lieutenant commander ability (row 2 slot 1) and puts K'Gan's lieutenant ability (Attack Pattern Beta I) into the slot that contained B'Ellera's old lieutenant ability (row 2 slot 7). In addition, most of the bridge officer abilities missing in step 8 are ones that I had retrained. I have no explanation for why K'Gan's Tactical Team I appears twice though.
Case 2
1. Log on to Bijin Senchou@frtoaster.
2. Go to the shipyard on Earth Spacedock.
3. Speak to the ship selection officer, Ensign Obin.
4. Select the R.R.W. Gomaru and then click "Ready Starship".
5. Beam out to the Sol System.
6. Arrange the space tray as shown in the images below.
Notes: I had claimed the R.R.W. Gomaru before Delta Rising, but had not altered the ship's equipment or configuration. In other words, the ship had its default equipment and configuration before Delta Rising. Now, the system seems to be trying to reset the ship's tray to its default configuration.
Case 3
1. Log on to Sarine@frtoaster.
2. Go to the Voth Ground Battlezone.
3. Put Integral Frequency Remodulator into row 1 slot 5, and put Emergency Shield Capacitor into row 1 slot 8.
4. Click "Leave Battlezone".
5. Beam down to the Voth Ground Battlezone. Integral Frequency Remodulator and Emergency Shield Capacitor have swapped places. Now, Emergency Shield Capacitor is in row 1 slot 5, and Integral Frequency Remodulator is in row 1 slot 8.
So I was having issues with UI changing whenever I go from ground to space. Was sent this fix by a GM.

Hello,
Thank you for contacting us and I apologize for the inconvenience this has caused.
As a potential workaround, you can try the following steps:
1. While in Sector Space, setup your desired powerbar/skillbar setup; BOFF stations; and DOFF active stations.
2. Save it to your existing/main Loadout.
3. Open your character window (press "U") and select your current ship
4. On the lower-left corner of the character window, click the button "Switch Starship"
5. Go back to your character window (press "U" to re-open it) then select your current ship
6. Wait for the "Switch Starship" cooldown to finish, then click it again to switch back to your main ship
7. Go back to your character window and select your current ship again
8. Select the "Loadouts" tab, and then save your current configuration
If this issue consistently affects your character (i.e. "all of the time"), please provide the character's name@handle, and a clear description of what exactly happens with the character, and under what circumstances this is seen, on this thread: http://sto-forum.perfectworld.com/showthread.php?t=1254191 Please be as detailed as possible, the information and character (that is affected all of the time) will help the relevant teams to reproduce and isolate this issue.
Thank you very much for your patience and understanding on this matter.
It did work on my fed character but not on my Klingon. I have no idea why this would be an issue, anyway. When I transfer from ground to space my UI tray changes, its that simple. If I beam to my ship the UI tray is not how I left it when I beamed down to ground. This bug is still happening to me as of this posting. I hope we can get this fixed, it is a very frustrating bug. I hope this helps.
All these "workarounds" for a problem affecting so many people yet no actual fix. Next I expect to be told I have to hold a half eaten blueberry muffin in my left hand and reset my skill tray on a Thursday after a full moon wearing a fake goat's head mask while listening to Michael Bolton's Time Love and Tenderness.
Incidentally now my SAVED loadouts are not re-loading properly. Things that were not even saved to that loadout are being put in slots. It's fun watching a problem get progressively worse. Almost like there was a bug in a program and people kept dumping new content on top it and suddenly more things are going wrong.
Ok, now it's happening IN THE MIDDLE OF A STF!!! Right in the middle of a mirror invasion suddenly I can't move and my weapons icon are different. I open up the inventory and see this
How am I supposed to be able to even play this game if this happens? I'm going to end up TRIBBLE my team because my ship decides to move things around?
So I finish get back to DS9 to file yet another bug ticket and after having reset everything in the stf it's all messed up yet again
I'm so glad this was declared fixed in those last patch notes, it has only gotten progressively worse at a faster rate.
I have noticed if I do not change my loadout in any way shape or form then nothing happens. The second I swap anything be it a console or item it all goes to sh**
Every time after change of any type of instance space, ground, pvp, pve

All skills drop of skill bar except for Photonic fleet/ Fleet support /Evasive Maneuvers
have to be replaced every time on all ships. And it is degrading to traits and doffs. replacing Battery's makes it more likely to happen.
Fix is to increase data space for storing player game data for ship layout, space trays and ground trays. Otherwise new items which get slotted will not be saved. Thus items seems to be fall out of the slots. If you can't recreate the issue in office, then have your staff log in from their homes and have many ships then try to add to the trays and see what happens. When you save layouts then switch out another ship.
Under my ROM Admiral, I had saved the layout, then switched to larger war bird ship. Everything on that ships was a mess. It had taken me like 4 times before layout on ship and the layout on trays would stick and not fall out when I jump to a mission or beamed down to a planet or station then beamed up to the ship.
Getting so fed up of this. It really does ruin any fun in the game.
Hal@khamseen
Move a power on the tray, beam down to any ground map, beam back up and power tray has reset. I've tried all the 'workarounds', even the one's which worked on some other toons do not work on this guy.

but my loadouts went ballistic when I configured my shuttle, using the same items that were on my big ship.

This is also happening on aesculapius@contactpsi and now seems to have migrated to another character somehow.

Holodeck patch notes: November 6, 2014
http://sto-forum.perfectworld.com/showthread.php?t=1287541
I have drawn colored rectangles to distinguish between different instances of the same ability.

http://i.imgur.com/nJEGOs7.jpg
The space tray now appears as shown in the images below.
http://i.imgur.com/p897nle.jpg
http://i.imgur.com/Y2Nk4rc.jpg
6. Arrange the space tray as shown in the images below.
http://i.imgur.com/lJEdPyt.jpg
http://i.imgur.com/STmXHfd.jpg
7. Beam down to Starfleet Academy.
8. Beam up to the Sol System. The space tray now appears as shown in the images below.
http://i.imgur.com/7vPtYXl.jpg
http://i.imgur.com/dw3owP0.jpg
Notes: I had claimed the R.R.W. Gomaru before Delta Rising, but had not altered the ship's equipment or configuration. In other words, the ship had its default equipment and configuration before Delta Rising. Now, the system seems to be trying to reset the ship's tray to its default configuration.
Case 3
1. Log on to Sarine@frtoaster.
2. Go to the Voth Ground Battlezone.
3. Put Integral Frequency Remodulator into row 1 slot 5, and put Emergency Shield Capacitor into row 1 slot 8.
http://i.imgur.com/aWv4LJg.jpg

Hello,
Thank you for contacting us and I apologize for the inconvenience this has caused.

Incidentally now my SAVED loadouts are not re-loading properly. Things that were not even saved to that loadout are being put in slots. It's fun watching a problem get progressively worse. Almost like there was a bug in a program and people kept dumping new content on top it and suddenly more things are going wrong.

I don't have problems with load-outs cause I don't have them never works for me, it was buged from day 1, i was able to use only 1 load-out.
I AM PISSED OFF AFTER 3 YRS. I STILL CAN'T USE OR CHANGE OR DELETE OUTFIT IN 3 OF 5 OUTFIT SLOTS ON MY KLINGON TOON !
[SIGPIC][/SIGPIC]
All skills drop of skill bar except for Photonic fleet/ Fleet support /Evasive Maneuvers
have to be replaced every time on all ships. And it is degrading to traits and doffs. replacing Battery's makes it more likely to happen.
Time will only tell!
Hal@khamseen
Move a power on the tray, beam down to any ground map, beam back up and power tray has reset. I've tried all the 'workarounds', even the one's which worked on some other toons do not work on this guy.
