That's the form to request the UFP signature, with the portrait and such. There's another thread titled Star Trek Online/Star Trek Infinte Space signatures:
The signatures we use don't fit on the Star Trek Online or Star Trek Infinite Space forums so we have an alternative signature for use on these websites.
If you are a Star Trek Online website user please make a post in THIS THREAD asking for a signature to be created for you.
I remember there being a thread somewhere else that listed all the STO Forum signature variants, like the gold banner underneath this very post, and the red banner underneath yours. There was a different form where we selected the signature variant we wanted with a bullet menu.

Join our Fleet were nearing level 2 on all tier's on our Main base and doing well on our Alt fleet base yes we have 2 starBase's due to our size as a fleet and our Glorius Klingon base is off to an amazing start!!!
Well we've held the Borg Back again there were many casualites but a few of us live on! We drew the line no further they were stoped thank's in part to our Klingon Brother's and Sister's for with out them we may have lost. But the BORG WILL BE BACK we will need your Help to stave off there advance's!!!
We've hit the maximum number of characters possible in our main Federation Fleet (UFP), and as such we are unable to invite any additional characters to that fleet. We're working to implement a solution to resolve this issue as soon as possible so invites may resume. We apologize for any inconvenience.

Yes we just maxed out the Main Fleet but our Reserve Fleet is still not Full and we will probaly start a Third one but we still use one FLEET CHAT between the {UFP}, {UFP}Reserves,and our Klingon fleet House Of Kular.
So just got to this website and apply and we will Fleet Forum PM you all the need info on ingame fleet joining.

Btw, if you're looking for an STO signature, make sure you request one in our signature area in Member Resources.
