If going from TFO menu (three arrows under the mini-map) clicking the Patrol Tab at top, then Mirror Universe tab on the left, highlighting Jupiter Gauntlet of either difficulty oftentimes launches me into Bringer of War instead. I've not heard of anyone else having the issue, so it's worth noting the characters I've had the issue with are Letar@Tom61 and Ixet'ika@Tom61
Now and then running either Patrol, finishing either Patrol exiting and re-launching the same Patrol, I get put into the same instance still completed with the Hail to Exit Simulation. This happens with either leaving via the Hail or the Warp Out next to mini-map, and with several seconds having been out of the mission (though, still less than a minute in most of the cases).
When launching via the Events menu, Patrols sub window, you can't advance the dialog with the F key until the Events window is closed, only by clicking. The sub window for the Patrols can be open and hitting F will work if the main Event window is closed, though.

It seems that it'll eventually autocomplete when that happens, as there seems to be some sort of hidden timer for just such an occasion. I've only got Elachi once, though, and it took a long while to finish up after I made sure everything was dead and then parking on top of Jupiter Station.
You kill all the Elachi as normal, and then... nothing for a good long while until it times out and finally hails you. I made sure to fly around to catch any potential stranglers that were out of visual/render range and found nothing still.
That just happened to me last night (or really, really early in the morning depending on how you look at it) for the first time. I also went hunting and found a few enemy ships wandering around way out from where any of the battles happened, but it still went on for a few minutes with nothing hostile on the map or visible in the 3D window. I wonder if some of the Elachi are jumping inside of one of the stations or something (that kind of thing used to happen in Defiance fairly often) and getting stuck in the geometry.
