I think you would have to hide your NPC and then spawn a "friendly" encounter (search for Guards) and apply your NPC's custome to the encounter. Not sure if there is a friendly encounter with just one mob in it though.
The projectors usually end up projecting their textures on ceilings and walls for me but I may be using them incorrectly, as to your question there are other blood splatter decors that you can place that aren't projectors which work fine.
