Spells can certainly happen in combat. 4e has some sort of healing surge mechanic, though most people don't like 4e and it's surge mechanic, personally I don't know much about it. Paladin's can use class abilities such as lay on hands.
Yeah. Translating that to video game form would probably end up being a huge disaster.

In short, Potions, Healing & Herbalism Skill, Cleric Spells, Druid Spells, Paladin Spells, Magic Items, and I could keep going on. There's tons of information on D&D out there. If you need to know more than that, please be specific on which you'd like explained better. Most of this, excluding surges, are already translated in Neverwinter in their own mechanics.
