This is my experience as well, once you qualify for rewards, RNG takes over from there and you play the % dice roll. I have absolutely no doubt this is how HE rewards will ALWAYS work, don't expect any convenience changes on this, ever (infact it was even nerfed at least once).
Not expected, just am HOPING with enough support in here, they will notice the unhappy players...
Its a SUPER low chance to get, OFF a super low chance of spawning (major HE)... And an even lower chance to get, off more common events like minor HEs....
I don't even bother to farm minors now.... After the latest nerf, they always give me 75 BI for great success... I might as well go dig up some BI, that would be faster. I don't understand why the HEs have to be nerfed twice so that the majority of the population doesn't get the gear by pure luck? If the content is hard and only a handful of people can beat it, like wow, and make the gear drop every time when one finishes the content, then it's acceptable, since at least there is a way to achieve it. What is happening now is just a pure disappointment.
I solo'd the Epic Bear Rider's Invasion (or whatever you call it) in Icewind pass last night. I am a GF, so you know how long and tedious that was, and there were many 'Oh s##t!' moments (I was popping Fighter's Recovery all day!).
My reward? 75 Black Ice and a Potion of Rubbish.
Before that I took on the Remorhaz hunt and I did not even get a potion.

PSA: You don't need to grind Spinward Rise for your Elemental artifact main hand if you have some AD lying around. You can craft it via the Tyranny of Dragon's campaign screen.
Resolving the issue(s) with the HEs is a multi-step process, IMO:
2a. Check and recheck the scoring algorithm so as to give proper credit for dealing damage, taking damage, healing, buffing, debuffing, and time involved in said HE - someone who has been contributing from the start, even if they didn't deal as much damage as a latecomer, should have their contribution given a bit more wait, (obviously you'd also need to check for AFKers, so maybe just use the time involved as a multiplier to their other contributions).
The top person of every class should be rewarded for their efforts. The CW, TR, GWF, DC, HR, and GF who participated the most should be guaranteed a purple drop. The hard part is the equation used for rewarding.

I like the top class is rewarded, however dont think should be 100% weapon.
There are alot of better ideas above than this TBH. Im not saying its bad, just that other suggestions are better.
A currency system would be awesome TBH. Add a new "currency" that drops from HEs (or chance to drop but a high one) and add THAT currecy requirement to ALL exsiting blackice gear from the vendor, ontop of that, add weapon and bracers to the vendor. DONE.
Now to get the full set you STILL have to grind HEs like crazy as well as farm blackice AND do the dailies for rep. Now its truly a set you have to farm for.
The Blackice currently takes only a few hours to farm enough to buy it all - arguably probably not enough farming required.
The Mainhand and bracers take TOO much time to farm...
So adding currency to HE, then having all BI base gear available for Blackice + currency (including MH and bracers) seems like a VERY good option.
