1) Immersion breaking issues.
II => Character Animation Overhaul: For instance: Beaming out results in players "typing on their wrist" for some silly reason. NO Star Trekkie ever did so. And this is only one thing. They either used a Tricorder-Comm or their Com-badge. NOT their wrists.
III => NPC voicing: many NPC do not even speak in missions. Sometimes they do, then they don't. Please fix voicing.
IV => Player/NPC voice-mouth sync: also been told by many and me, to no avail.
V => NPC pathing issues: several times asked, never really fixed.
VI => NPC (both enemy and player-ally) AI: for instance, incoming grenade. I get the feeling they want to headbutt it back, and when missed wait for the boom. They simply do not run away. Some go stand in a frikken fire and just die from STUPIDITY. Immersion breaking issue.
VII => NPC Ally Command system: I "need" to be able to tell my guy BEFOREHAND what to attack (in simplified form): =>Attack heaviest first/Attack Weakest First/Attack Pets First <= and make this a standard tactic.
VIII => Disconnect Map Swap: I can be for HOURS on end in STO, but changing a map (SecSpc to System or vica versa, SecSpc to Intern Map (planet or building) or vica versa, System to Intern Map or vica versa. The fact I NEVER get a DC when I am not map swapping tells me enough: it's not internet related. However, after a few times taking this up with support no solution has been found so far... Yes, immersion breaking as it destroys the 'fluidity' or actions, popping up an error and then log in screen.
IX => Map sometimes bugging out, waypoints not there, or autoplotting course failing.
XII => Spectres Mission speaks of Eta Eridani, while in fat it is the Donatu Sector!
XIII => many space maps have issues when collecting R&D Materials, TRIBBLE up the mission completely beyond restore. One must leave the system, and begin from 0. Needs be fixed ASAP.
XIV => Some missions have defective video movies, like the Cardassian Struggle arc.
XV => Some missions have bad audio, like in Operation Gamma, when you speak with Weyoun.
XVI => During combat, a lot of citizens simply ignore the fact of danger. Excuse me?
XVII => Discovery persons: NPC transport graphics are standard Federation beam-up, not Discovery, though i AM in a Disco ship.
XVIII => [T6] Fleet Tellerite Pralim Flight Deck Assault Cruiser has warp issues (graphical: stuttered flight) when going to Warp (system entering/leaving) with the Discovery character. Both seem to clash with there respective Warp graphics.

II => Several movies STILL inescapable.

You fail to understand: Borg Beam Up is not just the beam thing, but the motion as well.
Borg stand still before transportation, they do not 'wrist' themselves.

Something I didn't see on your list, you might want to add for immersion effects. Discovery BOFFs have rank limitations on some uniforms and on others, there is no rank assignment possible at all. For instance. If you put your Disco BOFF in a TOS uniform, the option for rank doesn't appear. In the Sierra uniforms, even if they've been promoted to Lieutenant, the only rank choices are none or Ensign.
I wouldn't rank a non-escapable cutscene as "immersion breaking". What would normally be immersion breaking is escaping the cutscene. It however is annoying, although devs stated not all of them can be redone due to the time being used for background workings. Would be nice though if it was cut down to the bare necessities.

Generic warp out bugs started a few months after launch. Dont remember them at launch. This is where the ship leaves the sector then does the audio sounds and flashes out and then the reverse where the ship charges to warp but coast in the sector like a 90 year old driving on the freeway.
Discovery beam outs they took a shortcut and have them tapping a combadge with a bright light so you cant see it. Sorry seen all of Discovery its a TOS hand held communicator but they hold it sideways sometimes "Gangster style" and the animation is in no way right.

Reputation tabs that show you your tier progression disappear and you have to tab to something else to get it back.

I => On some reputation ground sets, bridge officers cannot use the visuals, or some parts of the ground armor set visuals, in the tailor. An example of the first issue is the Temporal ground set. An example of the second is the Competitive Wargames ground set (chest piece visuals cannot be used by bridge officers, but the rest of the armor can be. It looks incomplete). This has been a problem for a while now.
II => The Jem'Hadar ground armor set's visuals don't include a full costume. You have to buy the Jem'Hadar tactical uniform in the C-Store for it to look complete, and not look like some adhoc garbage a Naussican would wear on Nimbus I
III => The mechanics of Undine infiltrator is broken. There is a problem with suppressing the fires.
IV => During ground combat in aim mode (B key) if u activate a skill while armed you go into a non armed state,sometimes your crew wont rearm themselfs and with risa costumes there is a noticeable color difference between the top half and bottem half of the characters backs.
V => You still can't take a newly minted Rom toon to New Romulus Command for your lvl 10 promotion, because NRC (supposedly) doesn't exist yet, without either a) sitting through the whole D'Tan/Obisek unskippable cutscene, and then going up to the front door, ( but there is nothing in game that actually tells you, that you can do this) or b) joining a fleet with an upgraded Embassy and going out the back door
VI => Almost all of the Romklink tailor options have 237 different colour palettes, some even between the left and right pieces! And most Romklink tailor options do not include greens or purples/violets which are absolutely necessary and "required" colours for Romulans and it's been how long since LoR dropped?
VII => You don't ever want to commission a new BoFF before making your alliance choice, because the game then can't figure out what outfit/s they can wear .. so you get almost none!
VIII => There is no "academy" on New Rom, and not even a Reman DoFF "representative" anywhere to be seen, let alone any other species - even just an "Alien" DoFF vendor/representative would be nice.
IX => The hastily added 'Specialist" BoFF vendor just has white BoFF's with the class/title, with no mouse over tool tip to say what they are, or what abilities/passives they have
X => You still, even to this day, cannot buy "basic" Equipment Upgrades from any vendor anywhere on New Rom.
XI =>Lifetime/Veteran players cannot get a "Romulan" veteran officer, so you must get either a Fed or KDF one, which means you can't put them in Romulan Uniforms.
XII => The Embassy Romulan BoFF's are gender locked.The KDF ones for Fed toons aren't

Otherwise, and this is a general issue outside any faction, SAVE for Disco, all are "wristing" themselves before being beamed up.

A Klingon Fed starts out as a member of Starfleet which is why they don't use their wrists. A liberated Borg of which ever faction is an officer of that faction which is why they use the beam out animations of the faction they belong to. The Borg are not currently a separate playable faction. Not in the same sense that the Klingon Empire, Romulan Republic, and the 3 different time periods of the Federation are. IF they were, I think you'd have a stronger case.

Before there was one big sector space map for all the Quadrants, the sectors used to be grouped into sets of 3 with a map change required between all of them. These groups were called "Sector Blocks".
The Donatu Sector was grouped with Archanis, Aldebaren, and made up the Eta Eridani Sector Block. These three sectors still makes up the Eta Eridani Sector Block, only usage of "Sector Blocks" in the game is out of date. I believe there are a few other older missions that still refer to Sector Blocks out there on the KDF side.
So, in a way, the mission terminology is correct to refer to Eta Eridani, only the usage of "Sector Blocks" is obsolete. And needs to be updated...not an egregious error nor immersion breaking, once you know why it is there.
Before there was one big sector space map for all the Quadrants, ...
When they make changes, they should add these in the whole as well.
Updating a part, leaving the rest just old, simply does create confuse...
There are a few out there that are not "too much" into ST, that see it is just that, a moment to sit back, mind blank, and let it just happen, not caring for ANYTHING but moving images and sound.

In my specific case first you see the ship jump away and disappear, then you see the "warp" hole forming.

You still can't take a newly minted Rom toon to New Romulus Command for your lvl 10 promotion, because NRC (supposedly) doesn't exist yet, without either a) sitting through the whole D'Tan/Obisek unskippable cutscene, and then going up to the front door, ( but there is nothing in game that actually tells you, that you can do this) or b) joining a fleet with an upgraded Embassy and going out the back door (again with no indication whatsoever that it's possible, for new players) - and it's been how long since LoR dropped?
Almost all of the Romklink tailor options have 237 different colour palettes, some even between the left and right pieces! And most Romklink tailor options do not include greens or purples/violets which are absolutely necessary and "required" colours for Romulans and it's been how long since LoR dropped?
And you don't ever want to commission a new BoFF before making your alliance choice, because the game then can't figure out what outfit/s they can wear .. so you get almost none!
There is no "academy" on New Rom, and not even a Reman DoFF "representative" anywhere to be seen, let alone any other species - even just an "Alien" DoFF vendor/representative would be nice.
The hastily added 'Specialist" BoFF vendor just has white BoFF's with the class/title, with no mouse over tool tip to say what they are, or what abilities/passives they have
You still, even to this day, cannot buy "basic" Equipment Upgrades from any vendor anywhere on New Rom.
Lifetime/Veteran players cannot get a "Romulan" veteran officer, so you must get either a Fed or KDF one, which means you can't put them in Romulan Uniforms.
Why are the Embassy Romulan BoFF's gender locked? The KDF ones for Fed toons aren't
