Workaround that I've had work for me and a few other people with this issue is the following.
Reset your tray to exactly how you want it, BOFF powers, DOFFs, everything.
Save it as a load out, either a new one or over an existing one
Remove one item from your ship, doesn't matter, just pick one
Reload the new loadout you just saved
Transfer maps
Return to space and check your tray
You should only have to do this once, but so far since I've done this my tray's remained fixed. I hope this helps people dealing with this bug, as this really is game breaking when it occurs.
Starting to have this happen now. However, it's only with a single power. Last night I swapped out my ensign tac for something else by mistake. I reslotted my tac and now his TT keeps unslotting itself. The rest of my power tray is fine since I haven't changed those out since before S9.
Was wanting to switch ships for a bit and fly something else, but I'm afraid that might trigger my entire loadout to unslot itself. Going to hold off on that since reslotting one copy of TT isn't horrible.
Edit: Now ST is unslotting itself on one of my other ships. Boo.
Mine is related to 2 powers. They just unslot. It cant be the BO because his others skills are fine. It doesn't happen all the time but at least once a day. Sometimes its on log in, sometimes after doing a ground map and then going back to space. There is no real pattern aside from it being the same two powers
Now IMO many issues are being cause by the way the entire system works. Modules shared with ships, Bank and Inventory. Seems like I have more issues when I have a full bank which is 99% of the time.
Often just going to ground maps or going to space I get a message that "All something something couldn't be loaded" but there is nothing different in my equipment or load out or tray.
once I somehow..still no clue how I did it..got things to stick, I tested every ship.
I have 4 of them, plus a runabout that rarely sees use.
steps to try:
1 arrainge tray/boffs to suit..save loadout.
2 beam down to starfleet academy
3 beam to earth space dock
4 beam up to ship
5 check tray, if all looks as it did before, mission success. if it doesnt work..repeat steps 1-4, and consider adjusting boff positions.
6 swap ship, repeat steps 1-5 as many times as necessary.
the reason for 3 transports is simple, you're forcing the game to clear the space map from active memory, and thus trying to force the tray issue to reoccur. if it doesnt, well you've successfully fixed a ship. please note some tinkering of loadouts/boffs may/will be required in order for it to save the loadout. once this is done however, it should stay. this may or may not work for you, so keep twiddling around with it till it sticks. just remember the 3 beamings.
this is what I tried but I am not 100% on the required mechanics, thus this may or may not work.

the only workaround i have found and marked in many topics is that you delete your shuttles/smallraft everything that is not a starship.. from that moment your power-tray will work.
so dear bugteam, id start the debugging on the shuttles as the 2x "active" ships that is glitching badly the power-tray according to my testings..
tho i dont even need any smallcrafts so deleting it wasnt an issue but sum fancy toons that bought ferengi shuttles or other non-reclaimable ones gonna feel bad for doing this

I have yet to make a ticket, but perhaps I should. I was doing this on my other VA, and tried a fix (move boff, save, etc.) and it worked. Now, it started on my ROM VA...but I noticed that my power tray goes wonky when I go from Space Dock to Defari Space. When I start doing the Defari Daily Missions, each time I change maps, the power tray changes and reverts, no matter what I try. So...perhaps it is because each day you do the missions, the map changes between 2 to 3 different things you have to do on each map. PLEASE guys...try to fix this, maybe go back to the one mission per Defari daily, not 10. Thank you.
^ Above works, but 5 steps per ship, per toons is a ridiculous amount of effort as a work around.
For the moment, devices are the 'only' issue, dropping them entirely when I switch maps.

My carefully constructed space hotbars go random when I go to Dyson sphere ground, then back to any instance of space. I search my powers out, and put them back into order, but when I go to Dyson ground to another space instance, such as and SFT or Dyson space, they are messed up.
This in only with the new Fleet Patrol Escort Retrofit. I have not noted this problem with any other ship. I guess I will test more to be sure.
I have tried the remedies posted above around SFA and ESD, and my hot bar will stay the same if I let it be, but refused to accept my placement... period.
Sadly, this has now rendered one of my captains unplayable. The entire hotbar unslots itself, as do my space doffs and several consoles. It seems to have no pattern on when it will all revert. Sometimes I'll beam from ground to space and it will be messed up. Other times it's perfectly fine. Upon entering a 20 man fleet defense, everything was fine. As soon as I warped out, everything unslotted.
I'm also not using the loadout system at all. After hearing how bad that system is, I've avoided it.
I'm not sure how bad it really is for everyone, but as far back as I can remember I've had slotting issues (and I've been playing for over 4 years).
Randomly various abilities will change position. Not all of them, just a few.
This is more noticeable when the tray has empty slots. All of a sudden a previously empty tray slot will have an ability I didn't place there myself, and the original location of said ability is now empty as well. I don't know if this is due to map transition, but during a battle after a series of key mashings, I'll notice it's wrong.
For example, not 10 minutes ago, I had two abilities move: ST1 and TSS2.
They moved from the C6 & C7 slots to C1 & C2, respectively. No idea how that happened. The only thing I did in between that was play SOI.
Today is the first time I used a loadout, and on a different character.
I am having trouble too....my Skill tray resets anytime I use the transporter. This problem has existed since Season 6 nearly 2 years ago. I really don't know why the skill tray has a lock tray checker when this bug will scramble it regardless.
From time to time I have the problem too, but my workaround I had for doff, does its job on the skills too.
I prepare my skillbar, save it. After that I remove an item from my ship and load the build one time. After that skills stay were they should be.
Sometimes things still mess up, weapons or boffs are removed or something like this. But as I can load the build without any problem, its just a second of work.

What finally worked for me was space map moving from sector to sector. After going the slow way from ESD to Tau Dewa, the loadout persists now... thank you!
I'm experiencing a slightly different version of this bug. All my Captain and Career powers stay where they are, but my set bonus powers, batteries, autofire selections, ship console powers, trait powers and also space-based duty officers ALWAYS reset without fail every time I go from space to ground and back. My doffs are particularly annoying because a lot of them are green or blue when I have purple versions back in the roster.
Ground is absolutely fine, no problems, I can slot powers, items, doffs, no problem, it's always in space.
I have a KDF Science officer (a male Klingon) who continually has two of his space tray button powers removed every time I get on him from another character or initially select to login as him when I first launch the game. I've put them back repeatedly and have tried to lock the tray with them slotted to no avail. It will also remove them when I change maps from space to ground and go back to space again. It's highly annoying. It seems trivial but after you have had to do it 50+ times it starts getting tiresome and unamusing rather quickly.
My Caitian character has become almost unplayable due to this bug.
It has only started since they became level 50. I have given up trying to do any of the later episodes as more often than not I have to try battling with very few of my powers available.
I am in the process of trying the various fixes suggested in this thread, with luck one of them will work so I can finally finish all the episodes.
Edited to add that none of the work-arounds solved the problem. I am either going to have to abandon the character until the problem is fixed or just keep them in space and not do any missions that involve transferring to the ground, which kind of limits the playability somewhat.
Here's the workaround I've been doing, that seems to work so far (and hopefully also serves as additional data points for the devs):
1: Customize your ship (can be done in any location or map).
2: Move to a social space map (e.g. Sol System) if not already in one, and there customize your tray slots and DOffs. (I'm still seeing saved loadouts still fail to persist when saved in sector space or a mission map.)
3: Save the loadout (new or existing doesn't matter).
4: Remove one item from your ship to your inventory (as nlindgren57 suggested a couple of pages ago).
5: Exit to sector space; once there, reload the saved loadout. This should put the removed item back where it was. (This is different from nlindgren57's workaround, where he reloads the saved loadout right after removing the item, and then changes maps.)
6: Check your tray settings; they should also remain as they were saved.
OK, first off, this doesn't make the game unplayable. Its still perfectly playable by using your mouse to activate your abilities. I know because I've been waiting for a new keypad to come in from best buy. And second, I haven't had this problem at all. Then again, I don't even use the loadout function. I also don't have a different setup for each ship. Nor do I macro multiple abilities to one key. I use one key per ability, and one ability per key. On top of that, I refuse to buy a windows 8 computer. Windows 7 runs the game just fine. No crashing, glitching, long load times, etc. This could also be from having the hard copy of the game.
Hopefully, the devs read this thread, and this difference can help them figure out what's wrong. Also, hoping it helps show that newer isn't better.
Noted this as a minor issue a few days ago and tried to identify the issue for me.
Map transition itself functions. But move from ground to space or vice versa and the trays glitch.
Using a Tac Odyssey from the three pack it always removes my Odyssey commands from my tray and slots the exact same powers in the same slots. (Not the ones I put the ship abilities.) The powers are Ramming speed, abandon ship, (And forgot the third sorry.)
Also I keep all my weapons to auto fire. But on transition the two right most weapon slots turn off.
Also noted all of my traits deactivate. Since this is a transition to a combat area, I cannot re-activate any of them to actually use them.
I have not used the loadout system.
Issues are since S9 launch.

I am having the power tray issues as well. It seems to only affect my newest lvl 50 and an older character that I swapped out a new ship on. I have tried the various workarounds but have had no success in keeping my power trays setup they way I want them. When I change maps, they are reset with various powers scattered in random locations on my trays. In most cases, the Bridge Officer Powers would disappear and the other powers would be the ones randomly scattered.
My newest character has 2 of the same loadout saved as part of one of the workarounds. I am unable to swap out either one. When trying to load either loadout, the loadout list seems to be caught in a loop and both loadouts are greyed out. Both loadouts will alternate as being the selected one, but am unable to load a single one.

Getting this problem on one of my characters as well. Go to a ground map, come back into space and the power tray has reset itself, every time. In addition to that, abilities from items such as the RMC and Azura comm code disappear from the tray as well.
Today I've also noticed that it's removing all my active DOFFs from duty too. It's only this character as well, none of the others are having any bother.
Stopped it from happening for the time being on my tac.
Equipped my ship how I wanted to. Created a new loadout for that setup. Unequipped a few pieces of gear and some boffs. Loaded the loadout I just made. Nothing has vanished from my hotbars for a few days now.
Still have the problem on my sci main, but I don't want to use loadouts on him since he flies a Wells. I can't dismiss and reclaim that in order to reset the loadouts on that ship.
