Avatars must be no bigger than 80x80 pixels (old forums used to be 100x100, but we can't use any bigger than 80x80 at this time) and 19.5KB in size. They also have to heed the forum rules found here.

Test - 80x80, less than 19KB... Ah! There it is! But only in postings in the forum, at the top of the forum I still only see the Starfleet communicator icon.
Now if only I could get it to let me use my old avatar...keeps telling me "upload of file failed" no matter how much i shrink the image or lower the quality.
EDIT: Apparently it actually took the image, but tells me that the upload failed anyway.
I had that issue, I think it uploaded it when selected and then we probably hit save but at that time the avatar had been uploaded and the form no longer had the location of where the image was.
