Spinning a dragon is what happens when you have aggro and run away from the bitey end of the dragon, causing it to chase you in circles, and turning its tail-slap attack onto your allies so they spend more time dodging than doing their jobs.
If you can't actually stand in the dragon's breath weapon, all you need to do is step right-then-left instead of always right, and the dragon will pivot within a limited angle instead of spin in circles, meaning your allies can hold their positions more effectively and the fight doesn't turn into a hot mess. Keeps the tail slap predictable and keeps breath weapons in a contained area.
OPs and GFs that are using powers that generate tons of threat but refuse to actually facetank the beasts particularly rub me the wrong way. Do not run around poking the dragon in the HAMSTER!

Most hated items:
- Synchronized alpha strike by multiple mobs in a pack on initial aggro (looking at you archers/huntsmen)
- Mob AI that goes to ridiculous lengths to get/maintain combat advantage
- Black dragon cultists that synchronize their transmutation into arpen godlings with white dragon cultist's spin/knockback AE.
- Terribad and misleading statistics listed/used for combine successes.
- Injury kits.
As a new player, the one thing that annoy(s/ed) me most is the too fast level progression (5-60). The game is actually fun to play while progressing through the main storylines, not yet the grind-and-repeat it turns into later. But you level up that fast, that you cannot even do all of Knox's main stories. On my first char I tried...until I was 12 levels above the quest level and could just walk through the zones being completely ignored by the mobs. (No fun.) With my second char I skipped half of them and it about worked.
Second annoying thing are the parts of the maps where you do 2 (or sometimes even 1) quest and then move on to the next respawn point (remember "collect one flower and move on"?). Hey, we are able to do 4 or 6 quests in one part of the map before being bored with it, easily. The graveyard is great in that regard. When you're through with it, you really have the feeling to have "finished" the whole map. (But then the daily grinding for dragon starts...) And then there are the maps where you get to a respawn point, get one(!) quest, finish it, get another one(!) quest that sends you to the exact same 3 locations, get a third quest that sends you to, you guessed it, the exact same 3 locations, and move on. (e.g. the final part of Mount Whatsitsname)
Third would be some of the quest instances. A couple of times I had it that I could more or less leisurely walk through it, handle all the encounters almost with just my at-wills and then being one-hit killed by the boss. Um, balance? Progression?
Bonus annoyance: Playing a Drow and realizing that there's not a single dialog line acknowledging your race. Being called a surface-dweller in Underdark is "funny"...not.

The CW transmute set from Protector's Jubilee is more like a long dress with a split skirt. Fully covers your torso as well. You might like it.
Probably have been many threads like this before but as content changes it can result in a lot of fun answers. What in the game do you hate more than anything? What makes you want to scream at the screen? And I do not mean bugs, or the game itself. This is meant to be light hearted and fun. For me, I HATE Redcap Witherers. I mostly play a paladin and there I am just about to blast of Divine Justice and nope, action points are gone and I know that little bugger is laughing at me. It drives me crazy.
So what monsters or traps or other such things drive you crazy? And make sure you keep is fun!
Refining, plain and simple. Especially when it's as unimaginative and arbitrary as it is in this game.

Server Response issue... that lead to dc's.
I have used tracert and the readings were optimum... yet I still get hosed.
40 dc's in one 2 hour period. So bad that I gave up playing the game for a few days.

I know you mentioned that you don't make much use of the fashion tab, but the Icewind Merchant bottom is rather sensible pants, (though still retaining a skirt of sorts):

Well there are a whole bunch of them, but I'll try to keep it light
I absolutely hate it when my character doesn't dodge right a way, and instead takes a step or two, than does the dodge. Another thing that drives me crazy is when I use a potion and I don't get healed, but the potions go on cooldown. Aaand one more thing, if I may; I passionately hate clipping on gear, weapons and hair.
I hate dancing with Lady Luck. She always steps on my toes.
Not what I was getting at, as I am aware of that. I'm just annoyed that they get to keep invincible block.

Result: You start you power blast - Bluey starts his block - Bluey has the block up - your spell finally, after thorough preparation, discussion, and arbitration decides to put up an ETA when it'll eventually fire - attack goes to damage point nirvana...
Since the OP wants in-game annoyances, the thing bothering me most there is that the community refuses to get along. So much hate and elitism..
Special mention to all those wee little 1337 _____ that feel the need to pick your single pickup quest objective after you've wheedled down the mobs with an HR or GF (pre-Mod-6) or... ...and then proceed to fast-wipe the neighbouring mob and pick that objective, too, before you can even get there...

If Cryptic really wished to do something about that, they'd either hire some moderately skilled person to police the game, or purchase a third party software package or service, to grab the problem at the root. But it's far more convenient to have it as a pull-out-from-the-hat scapegoat to blame whenever and for whatever, and nobody will ever have the slightest mote of doubt. Heck, they don't even have to take that part themselves...

Things that should be and are not
1) no cost to keep the look you want
2) No grinding more story options and paths
3) Classes should be closer to the books
4) Races not locked behind random drop boxes
5) mounts usable at first level and up grade able through training with a skill (intern costing items like feed, and cost of money for help ect..).
6) Things should take time to do not a grind to do and cost is fine as long as not excessive.
7) Monsters should be fought not used as mounts (gelatinous cube, howler, nightmare (evil only) unicorn (virgin female) ect..)
8) Death should cost something not be an inconvenience, this walks hand in hand with the difficulty being too high which means he who has the money wins because scrolls of rez and stones of instant health are way too powerful.
I know its a video game and we are here to have fun and they are here to make money and the two hardly ever go hand in hand. A sub cost would maybe resolve some of it but not all because if I pay for it I don't want to loose it and that's why this game is in the state it is. Because you cant't take away something some one has paid money for or make them loose it. So mounts and companions and every thing but consumables are there for ever, unless god forbid you delete them. Any way nothing will change in this game because its ran on those willing to pay for the shinnies and the wallet is mightier than the fun.

Such as not being able to vote kick in skirmish now. just lets people sit at the fire or allows automated characters to farm.
