Two groups of enemies seem to spawn inside the terrain. Here is a screenshot:
The enemies are untargettable, and my Chultan Tiger companion teleports from all around the map into the same sewer pipe out of combat.
Secondly, the blue area indicating where a box is shows up only for 2 out of the three boxes, but all three do spawn in places where they can be reached.
