Following the patch changes to refining I did the following:
Unslotted the existing item enchant - paying the 2g50s.
Slotted a new enchant, but didn't yet save that enchant into the slot.
While the enchant was in the slot but not saved I refined it upgrading from rank 6 to rank 7, using all the materials involved and three green wards. The upgrade was a success.
I then saved the enchant into the slot, all the upgrade was lost and I was left with the rank 6 and all the materials gone.
Secondly: as above, I then (I have to admit accidentally...) this time saved the new enchant into the slot on the item, but did not close the window before further refining it and the same thing happened, again leading to hundreds of lost R3 and R4 enhants to refine it up and the other reagents.
In oredr to get the refining to work it seems that you have to save the new enchantment, close the window - re-open your character slot and then start a fresh process to refine.
Frustrating, and I apreciate that not many people may have done it this way but I did this by mistake and the laws of internet suggest if i did it then others will have aswell, but I thought I would share this so that some poor soul doesn't do the same thing with R8 or higher in which case I may have cried actual tears.... Haven't done the first thing you are reporting, but that sounds pretty major.
For the second thing, I have done that before but not lost any progress, so not sure whether you did something special there

Oh gawd, still? A few people including myself reported this from preview.
Seems that whatever enchantment you initially slotted is what will get locked in when you hit that "enchant" button, even if you refined and upgraded it with the window still open.
I've noticed for the most part, the refine button appears to be greyed out until you have actually committed the enchantment, but under certain circumstances it shows up again (opening other interfaces?) and that's when this can happen.
