I have played a Rogue and Cleric now to about level 25 and so far I have found I have a ton of unused potions and have not even fallen once on either character from death or whatever the in game term of death is for Neverwinter.
When I use companions I rarely even take damage now either.
Also when I run my Cleric with parties many have their companion out for healing so although I do some healing it is entirely feasible that parties can run without healers altogether as far as I have seen.
So is that it or does it get more challenging outside of the modules that players have created that offer a difficulty scale? Do dungeons require a specific party composition as time goes on and we get to max level? Is there a reason to even get the best gear in game that pertains to skill increase?
I have been bouncing around from MMO to MMO trying to find one that actually offers a challenge and is not just a bunch of mini games of player competition and I am wondering if this one gets any harder than what I have seen?

Level 25 indicates that you haven't even gotten past the Neverdeath Graveyard questline yet. Helm's Hold and beyond, you may find that the content is more challenging, I think.
All bets are off for anything where your level is significantly higher than the monsters' levels, which is why Foundry can present more challenge since monsters will always be at your level.
I found the first real difficulty on my first character (CW) to be around Pirates Skyhold or Mount Hotenow.
Pretty much everyone will tell you though: PVE leveling in this game is really not terribly challenging.
The real challenge comes after you hit 60 and start running epic dungeons.
The game difficulty increases quite a bit at the higher levels and some of the Epic Dungeon Delves are very difficult indeed. Don't be lulled by the easy level ascent early on!
low level solo runs dont need much of those potions, only used a few with bosses.
yes, level up can increase more challenged "trash mobs", those solo questline are fun.
there 2 factors you should know.
1. check your equiptments, if mobs can hit you hard, then your armors and weapon wont help if they are outgrowned, get upgraded set that is near or on top of your current levels.
2. watch for "patroling adds" when if you find in tough spots, pull them to semi-cleared areas, or use enviroment to your favorable odds, if you have companion, make sure it is better quality.
stay focus, if not and you might end up drinking more.

It can get very challenging very soon:
As soon as level 30 you can run lair of the mad dragon, and it's very tough to do at level. I'd say something like 90% of queued instances fail. Performed manual entries that work together will have higher success rates, but even those often fail.
And really all of the 5 man dungeons up to 60 offer some challenge.
The solo content is all quite easy tell around 58, then it has a couple tough solo-dungeons if you haven't overgeared.
Epics are all quite challenging unless you're in a very overgeared party, and even that that really only happens in a couple tier 1s (Cloak tower, cragmire).

The real game seems to start around level 60. However, give the mad dragon dungeon a shot once you get to it and you'll see a bit of a spike. And then, at 60, try epic mad dragon or epic dread vault - you aren't going to win unless your team is amazing.
