I've outlevled Lair of the Mad Dragon, and as such, for some silly reason, I cannot see it in my list of dungeons that I can queue for. First off, is there some other way I can queue for it? I've spent the better part of an hour researching and looking for exactly where the entrance is, no luck.
I believe the only way is to queue at the dungeon entrance itself. If you want to experience it but can't find the entrance, I suggest getting to 60 and doing the Epic version;)

You literally can't, not until the level 60 versions.
I outleveled a lot of the skirmish and dungeons while leveling since the queues took so long.
you can do any dungeon you want if you enter the manual way. you just cant queue for it. the entrance is near the top of the map. there is like a little dip at the bottom part of all that green. the door is somewhere in that dip.

Can I queue for it at the entrance? Or do I have to go with a party ready to do it?

I love this game, and that is one horrible design choice. Horrlble. I was never informed of any of this, nor was I even informed of where the entrance is. Can anyone tell me where it is, exactly?
First off, I'm aware that its possible to solo or enter a dungeon with a group via its world entrance. Can this still be done at 60, for the normal versions. Or will it always default you to the epic ones?
Secondly, do the epic dungeons still count for map achievements for area completion? For example, If I wanted to complete Helms Hold, would I need to do normal "lair of the mad dragon" or would doing the epic version also work?
Ive been skipping dungeons while leveling in the thought that id be able to go back. Either solo on normal, for the lower level ones, or with a group, on the higher level ones. And still be able to complete everything. It also seemed a good way to get familiar with the various dungeons before diving into Epic encounters. Now, im wondering if that plan is still viable.

Normal and epic are tracked separately for achievements.
I've read about soloing normal dungeons being one of the things higher levels characters do to amuse themselves, even at 60, so I think you're fine.
Oh, and seen zone chat where 60s come to visit and offer to take the newbies through lickety split.
