Fireball Misfire

I was running my wizard through the stronghold for influence.

I have noticed this before, but today it stood out. I have Fireball on Mastery. Several times I cast it. The character goes through the motions. The power goes on cooldown. No damage is inflicted from the power. I had the combat log up. There was no indication in the log that the power had been used.
I'm not looking for forgiveness, and I'm way past asking permission. Earth just lost her best defender, so we're here to fight. And if you want to stand in our way, we'll fight you too.

    How frequently does this happen and does it happen on other maps?

    On a guess, fairly often. Not just on the stronghold. I don't normally run with the log up.

    It behaved the same way in Spinward Rise, The Well of Dragons, and the Thayan Magical Shelter.
