In the Vault of Stars, in the maze, mobs spawn inside the arenas. Very often there are two mobs of the same type spawning in the same place simultaneously. That does not seem intended. The same happens in the arena on the way to the first boss.
If the group dies from the mobs in the maze and someone resurrects with a scroll (that seems to be the cause), the maze gets bugged and the campfire at the end cannot be activated. To activate it, we had to go through the maze two more times.
The timer in the maze can run out due to the server lags, when the group has to wait too long for Lumi to activate the door or for the next wave of mobs to spawn.
After dying on the 2nd boss, some players get resurrected at the first campfire (at the start of the dungeon). Porting out or rejoining the queue teleports the player back to the right campfire.
After completing the dungeon in hardcore mode, the vines can trap the players (this happens a lot due to their location). /killme solves this problem. There is a similar issue with the exit portal in LoMM.
