Infernal War Machine HE still bugged

Activated this HE, defeated all enemies present until it said 3 of 4 groups...no last group in sight. However, something invisible was still attacking. It was using sweep, infernal quill, melee attack and dazing fist. HE failed because we couldn't see or target it. HR was able to do some damage with aoes, but GF aoes never hit.
