Fireball issue

On PC:
Sometime when i cast fireball, the power is expended and cool down starts, animation plays but the fireball does not detonate or do any damage.
This has made some fights far more harrowing because i suddenly wasted my best power without it helping me at all. this has happened a few times but since the launch of mod16 but it was never all that bad. But on june 1st, it happened so often, maybe 4 times in a single expedition that i think it is time i said something.
