Sometimes when I click on a destination in the map and then "OK" for Auto Travel, my ship merely engages the throttle and flies in the direction it's already facing indefinitely. It seems like it happens the most when I first log in to a character. Then after I rezone the problem usually goes away.

I usually set the destination and open the DOFF window and expect my ship to be there once I close it. For a while I was thinking the connection to the KDF Fleet Starbase was broken (that is where I was usually flying to when I notice I was not stopping).
I really haven't isolated it.

It locks at 1/2 impulse and just glides you in whatever direction your ship was facing at the time auto travel should have activated. And one just flies right by chosen destination.

Yeah, I had that happen to one of my characters after doing an episode in the "Cardassian Struggle" list...."Second Wave". But I don't spend a lot of time doing missions....so, I notice it more when I am DOFFing.
So, on DOFFing it seems to have something to do with switching maps.

While, I just started noticing this with Season 13 patch.
It does not happen very often and difficult to reproduce to even make a bug report on it, anyways.
I am just going to assume: once in a while the Auto Travel gets mixed up data and the command does not (or can not) finish completely. And best to just re-send the activation if one is drifting away too far off course...or take over the helm, and do it manually. LOL!

Were you seeing it tied to "Transwarp" or map changes, too?
I still can not get it to reproduce reliably....though I still see it often enough to make me go, "Hmmmmm...."

To reproduce the bug on a KDF character:
I am almost always playing my Klingon or KDF Romulan.
My Fed characters could be jettisoned...except I use them for storage.

I guess, I don't use transwarp other wise....next time I will just fly to the edge and use the map transition from Alpha to Beta and see if that makes a difference.
Same thing: Stuck at half throttle, going in the direction ship is facing when warp to the Sector space map. The Auto Travel does not set the trajectory to the destination properly the first time after transwarp. Selecting a second destination, "breaks the spell", so to speak.
Edit: Just now, completed "Mirrors and Smoke"...this time Transwarped from Gon'cra Nebula area to Defera....as in went from Alpha Quad location to another Alpha Quad location. This takes me to Defera System Space....then used the warp out button. Then used Auto Travel to DS9...did not work. Changed destination to Bajor, that didn't unlock it. So, I picked the Vlugta Dil Mine that worked...then reset the course to DS9. (I am guessing Bajor and DS 9 are the same location/transwarp destination?)
So, it is attached to the first Auto Travel destination...after using Transwarp to System space then going from System space to Sector space. ???? Probably goes the other way too.... using Transwarp to Sector Space destination from System Space.

Edit: When I was parked outside Iconia, I believe I was facing in the direction of the Azure Nebula on the Sector Map.

This is what I discovered on a Federation character. After warping out of the Defera System, my ship is facing almost straight toward DS9 and Bajor. If I auto-travel to DS9, the ship initially seems to travel in the correct direction. It travels at full speed until it gets close to DS9. Then it slows down to half speed. It will continue flying at half speed straight past DS9, unless it bumps into a planet or moon in the Bajor System. In that case, the bump throws the ship off course, after which it will correct course and correctly navigate to DS9. Pretty much the same thing happens if I auto-travel to Bajor. The only difference is that the ship will slow down from one-half to one-quarter speed when it gets really close to Bajor. Once it gets further away, it will increase its speed to one-half again. It will fly straight past Bajor, unless it bumps into a planet or a moon, forcing it to correct course.
I don't believe the bug is specifically tied to transwarp. I can reproduce the bug without using transwarp. I believe what matters is your starting position, the destination you select for auto-travel, and the direction your ship is facing. The question is how sensitive the bug is to these conditions. For example, is the bug still reproducible if your starting position is slightly different?
Though you are correct...from what I recall.....most of the time the ship is already facing very close to the direction it needs to be heading. Example: coming out into Sector space from Defera System....your ship lands facing toward DS9/Bajor. From Qo'nos System....ship comes out to Sector space facing toward KDF Fleet Starbase, I believe. And transwarp to Azure nebula...is in sector space facing New Romulus. That is how I was testing, though.
I should see what changes if I move auto travel locations to the opposite direction.
Auto travel when working correctly....ship is at half throttle as it orients itself to the trajectory it needs to head (I.O.W. turning)...then ramps up to full throttle once it is facing the correct direction. Then, throttle turns OFF completely as ship comes to a stop at the destination. (That is how I learned, turn down throttle to turn better in the game...LOL!)
If it isn't transwarp, then transition to system space immediately....though I never seen auto travel get hung up until I use transwarp. (At least, not YET.) Something is still getting incorrect data about orientation of the ship...because it never sets the ship to the proper course and never goes to full throttle. Like it is waiting for confirmation or something. Confirmation it will never get because: ship doesn't have the correct heading or never got set to the proper heading.
BUT it doesn't do it all the time....because it did not do it for me today. At all.
I am ready to call it lag or data loss causing errors at this point.

Yes, that's what happens when auto-travel works correctly.
I don't believe it's necessarily tied to map transitions either. I think it's dependent on position and angle. Transwarps to sector space probably bring your ship to the same spot each time. If I remember correctly, warping out of a system puts your ship close to the same spot, but not necessarily the exact same spot. It might depend on what other ships are around; I think the game tries to avoid stacking ships on top of each other.
I might be able sit there and try it next weekend while I am doing laundry.
But Mirror Event for Lukari marks may be in my future. Sooooo....probably not. LOL!
BTW....I got hung up again. Transwarp to Qo'nos System...went and turned in contraband in First City....then did Recruitment at KDF Academy....then went directly from KDF Academy to Sector space outside Qo'nos... Auto Traveled to I forgot where...stuck at half throttle going a weird direction.
That's a new one...that part about the First City/Academy.
So...you may not think it is tied to Transwarp....but I think it is.
Though...may not be tied to map transitions on Ground??? Or something like that???
Add: In other words, I don't think my ship would have got hung up on Auto Travel...had I flown directly to Qo'nos in Sector Space, instead of getting bored of waiting and hit Transwarp there.
