If you can find it, Robert Wise's Director's Edition DVD pretty much fixes all of that. Sadly, the new effects were made for home video resolution and Paramount doesn't want to re-re-invent the wheel for 1080p/4K blu-rays.

Happened to me too. LVL 50 toons. (FED eng, KDF sic) Talked to the vendor, I saw that pop-up loot notice, and the items never made it to my invintory. Quest completed, so the vendor isn't trying to give me anymore. Not in overflow bag; logging out and in didn't change anything.

Any new costume pack definitely ought to include the enlisted/cadet variants, as well as the security uniform... or perhaps include a rankless armor equipment that uses the security armor visual. I have not been able to reproduce this apparent error after changing my log in. Looks like there's not actually a bug here.

I do, but missions remove items from inventory when the assignment is started, not when they are turned in. My contraband disappears even without starting new DOFF missions. EDIT: Though it would be worth a troubleshoot to see if there is a bug that causes the turn-in of those missions to remove contraband a second time.
