The upgrade was NOT working as intended on Universal Kit Module - Fek'Ihri Chaos Blaze, as it kept using the upgrades and showing the possible result as Epic. After 10 Phoenix it went to UR, after 60 more, still not Epic.
As can be seen in the first screenshot, the "Item Has Obtained Maximum Quality".
Most items will upgrade to Epic, but most Kit Modules that won't go past Ultra Rare for whatever reason.
Usually, when something reaches its' maximum quality & mark, it won't accept any other upgrades. However, since most kit modules don't go to Epic, they will still accept the upgrades and show the possible result despite the message saying they have reached their maximum quality.
It's definitely misleading and needs correcting.

The fact that the message disappearing when the quality upgrade bar hit 100% doesn't mean it no longer applies. The item just will not increase in quality.
