"the doctor" was nickname due him being the only member of the medical department left alive, his actual name (or actually designation) is "Emergency Medical Hologram mark I", a role where he would more likely perform the duties of a Nurse or a Medical tech rather then those of an MD.
More over the name "Doctor" refers to one very specific and updated Mark I (more of Mark 1.5 then pure a Mark 1) that served on broad USS Voyager, what we get is a stock EMH mk 1 who wouldn't have those upgrades nor would they have the name "Doctor" either as that's specific.
Remember how differently the EMH mark 1 on board the Enterprise acted when activated by Crusher, we see only for a short time but it's that it's very different from the Doctor (aka Voyager's EMH mark 1).

Which could be an in-universe reference to Bones (or out universe one), again think about what role would an EMH fill normally in emergency, you'd probably have enough doctors but would be short of nurses or med tech to assist those doctors and telling a trained starfleet doctor "You're gonna play a nurse now so that Hologram can be a doctor as we made him refer himself as one as joke once" isn't gonna go well.
the EMH can any of those 3 a doctor, a nurse or a med tech based on what is needed, it is after all an AI program with a hologramic avatar so it can be everything the main computer needs it to be, that said the roles it's gonna fill will more often then not be that of a Nurse or a Med tech hence the "nurse" classification in-game, now if the game allowed them to be more then 1 classification they'd probably be "counts as both a doctor or a nurse" as far as classification goes but the nurse classification isn't incorrect.
The EMH was mostly there to support the rest of the medical personnel. That would make the role of a nurse more appropriate in more situations, even if it may not have been the only role he could fulfill.
The quote from First Contact is just a reference to McCoy - at least, I never interpreted it as being anything else than that.
I must add though:
The question remains whether the The Doctor doff is Voyager's Doctor or just a regular EMH. Since he's epic quality, it wouldn't be strange to assume that he's actually Voyager's Doctor.
In that case, one could argue that giving him the function of a Doctor doff would have been more appropriate. Then again, our ships probably have fully fuctional and well staffed medical departments - so that would bring us back to the observations, considerations and conclusion as stated in the first couple of lines above.

Not true. Adet'Pa is a DOFF classified as a Projectile Officer, but has the EXACT effects of a CONN Officer. Designation and function do not necessarily have to match in a DOFF.
And I agree that the Epic DOFF "The Doctor" should be classed as a Doctor, regardless of function ability. As an Epic chatracter, he probably is Voyager's EMH (or at least a copy), especially since he is actually named "The Doctor".
Speaking of which, Star Trek (for all it's virtues) has a lousy history of doing software backups. Heck, they should be doing TRANSPORTER backups! Ensign Redshirt just got killed on an Away mission? No need to write that painful letter to his mom, just beam up 180 pounds of rock from a passing asteroid, and use his last pattern (instead of the rock's) to rematerialize it. Tragedy averted. You're still converting the same amount of mass to energy, you just swap in the other pattern to turn the energy back into the actual form you want. After all, what's the replicator but a smaller, simpler, lower-power transporter? Your dirty dishes end up as someone else's Lobster Thermidor or Plomeek Soup. Same principle.
The moral of the story: responsibly back up your EMH's and your transporter patterns. Your moms will thank you!

The problem with the transporter pattern backup idea is that if you did it using a rock or whatever you would get a corpse, not a living person out of it. In TNG they revealed that transporters have separate channels for the material body and the "energy body" (for all practical purposes the soul but avoiding the word as usual for Trek) that recombine at the destination to form the living person.
TOS itself implied the same energy-body thing (though without actually saying it) with the mind storage spheres Sargon's people used in Return to Tomorrow, the body switch in Turnabout Intruder, and the problems Kirk was having when split into two bodies in The Enemy Within.
On the other hand, they don't explain why that pattern buffer trick is not used to heal still-alive people from extremely grievous wounds more often.
