After the first time-out when queuing, there's probably a 50/50 chance of the Accept/Reject prompt not appearing in the side-bar menu. The only work-around currently is to jump to another instance to get the prompt to appear, but usually the countdown will still time-out, and this workaround doesn't work on transferring to another map, only another instance, which doesn't help if there's no other instances running. If the time-out is a result of someone not accepting or someone rejecting I'm not sure, but I would hazard a guess that the queue system isn't allowing for a replacement to queue like it does on the PC.
Other than this, there is nothing observable that I can relay that will help.

I have not tried switching map instances.. Changing character and immediately logging back in with the same character will bring back the option to accept/reject the current queue.
this is just one more example on why consoles need a tribble server to test the stuff out before it breaks the game further.
it seems like every time there is either an update/patch or expansion that comes out for the game it break the it more that it fixes it.
When is the next update? Who knows. I just hope it's really fixed on the next update patch. I have been switching instances to join queues since the launch of VIL. Frankly, I'm getting sick of queues now.
Don't know when the next patch. I think @engineerb4#4806 could have been referring to the patch that hit the week prior. This current post is in response to Kael asking for more detail after I asked him about the status of the fix on last night's Livestream. They were originally convinced they had fixed the issue, resulting in the prior 'fix', and in giving them this extra detail and supposition, it should help them 'replicate' the bug.
