This seems to be a growing problem as a fleet mate & myself have the same issue.
My klingon got the MK XII honorguard set before S7 launched, but if I go to the tailor I can't select the helmet for it, yet I can select some of the other pieces for it.
A fleetmate had 1 piece of the Mk XII set before S7 and got the other 2 peices after ... he's ran all the unlock project and still can't select the new costume helmet.
If you have a M.A.C.O./Omega Force/Honor Guard uniform that should offer a helmet, you can wear it. You have to change the "Head type" category (on the top of the page concerning your head details) to your helmet.
Like I can select the helmet on my vulcan but not on my orion. Even loading a previously saved helmet costume will not unlock it (just makes her bald). I can do a screenshot if you doubt I know what menu to use.
From what I hear gorns are bugged too and probaby others aswell...

I have sent pictures of this issue to their technical team on more than one occasion.
I have the older mkxii and mkxi maco and omega ground gear, had all but the infected elite optional done before season 7.
Season 7 launches, and i managed to complete infected elite optional for which it confirmed with sound and checked it off in accolades, but did not unlock the omega shadow force operative and the maco elite commander which are associated with the completion of elite missions and their optionals.
Sold and rebought back all mkxi and mkxii maco and omega ground gear with success, having the game make a sound confirming each piece as if it was the first time aquireing them, so the game is aware of the gear being reaquired.
Completed all elite optionals 2 more times each without any sound confirmation from the game, therefore it doesn't recognize these as being achieveable again since they are already checked off.
Did all omega reputation projects available completing tier 5 without ever having any uniform projects available except assimulated maco uniform.
However type 2 omega and maco uniform is still available to me to use, even though no uniform projects were ever available. This uniform capability was made available to me before season 7 and is still available today, so the game and tailor recognize i have the right to type 2 omega and maco uniform, just not type 3 because it doesn't respond to my achieved accolades nor offer any type 3 uniform project.
These exact photos were sent to their tech team twice now without any real support help, the only changes to those accolades thus far is the completion of maco team specialist, vorn distinguished service medal, and the maco assimulated accolade.
Well, throw my hat into the arena, cause the bull bucked and all, as I have completed everything else STF accolade wise, and am wearing the mixed MACO ground set, and for all of it, I still do not have the OMEGA Mk XII set accolade, despite check marks shown on the screen for the mk XII OMEGA set. Everything else is complete.
I can only presume, as I got my OMEGA Mk XII rifle from a CGE run long ago, prior to the rep system, that the 'old' gun, and the reputation shield and armor, don't have the same coding or such. Would be nice to check by replacing a current project... oh, yeah... anyway...
same happening to me, as it seems, the reputation system is glitched and doesnt recognize the "maco commando" accolade from people who got it before s7, so we wont be able to get maco elite commander accolade ... i wont buy the gears again, ill just wait for this issue to get resolved

This is happening to me as well. I noticed it after I bought the mk xii pieces I needed. I had the MACO and Omega mk XII weapons prior to season 7.
I have all the gear X, XI , XII
I have every Accolade in both stf accolade catagories
I have run every project
I have adapted Maco coutume options and type 2 costume options
I do not have type 3 costume options There not even listed at the tailor
I have all of this on my other two toons

Would have gotten the full set for my klingon to the old way. Alas.. One day.. one day.. But there to busy with NWN now.. And after playing that Beta... this is starting to become troublesome..
Helmets got fixed on tribble. The Chest piece alas is still bugged. Send a new Email regarding this to Stahl. Lets hope he will look into that one to.
Sadly I cant comment on Accolades for STFS. But I would suggest heading over to tribble and copying your toon. See if it works there. Current patch on tribble seems to have solved alot of issues. Still. Leaving the issue that was annoying me the most open. But it gives me hope that that one will be fixed to with patiences and presistence. Still thank you devolpers to fix the bug partly. Lets hope you can tackle it completly
To view your Accolades in game, open your (J)ournal and tab over to the Accolade section, then scroll down to the STF sections.
If what you're saying above was interpreted correctly by the system, you should have an old version of the Mk XII Set Accolade who's descriptive text mentions both a Title AND a Costume Unlock, and a new version of the Elite/Optional Accolade which mentions only a Title.
Would you mind checking that, and getting back to me? I'm not having much luck reproducing this error locally and need more information.
