Ship weapons fire...but don't.

In combat, enemy firing on my ship. Target enemy, weapon icons are highlighted (in range and arc).
Fire. Icons dim, cooldowns (like if attack buffs are triggered) activate, no weapons fire (no sound or graphics), no damage in log, nothing.
Wait for cooldown, fire again. No weapons fire, no damage to enemy (or MISS) in combat log.
Happens about 2/3 of the time which makes combat very very hard as your ship is being hammered while you're trying to get a fire solution. And then when nothing happens when you keep hitting the fire button (mouse or keyboard) not very fun.
This used to happens with torpedoes about 25% (75% if they were High Yield) of the time. Now I come back after a while and find it happens to all weapons almost all the time.
