In one area of one ground map, Anchor of Gre'thor does not work

What the issue is: Anchor of Gre'thor's hazard immediately despawns on this map
What steps you already tried when attempting to resolve your issue (if applicable): Tried using it multiple times, on different enemies, with different other kit modules equipped, but AoG always misfires here.

In one specific part of one ground map, the winter event Engineering Kit Module - Anchor of Gre'thor does not work. When used, it goes on its full cooldown, but the fire patch and chains only appear for a split second before vanishing immediately, instead of lasting the full 10 seconds.
The ground map in question is in the "Terran Gambit" episode "Firewall" phase "Warped", on the Terran station in the mirror universe, only after taking the turbolift down to the lower level (where the archive is, with the Liberated Terran Exocomp). On the upper level of the same map, and in all other episodes/TFOs/etc I've tried, the module works fine.
In this screenshot, you can see Anchor of Gre'thor (middle tray, far left) showing a 13s cooldown, out of its total cooldown of 14.3s with my character's Kit Readiness. So it has just been used, and yet you can not see the fire patch in the scene, because it has already disappeared after just a moment.

In the very first ground phase on the prime universe Federation station, initially Anchor of Gre'thor does not work.
But, after activating the reactor and enabling internal sensors to restore the minimap, then it works again.
After taking a turbolift to the brig to meet Leeta, it still works.
But, after taking the second lift to the Lab (where Mirror Kuumaarke escapes), it does not work again.
But, after beaming up, traveling to the mirror universe, and beaming down to the upper level of the Terran station, it works again.
But finally, after rescuing De Soto and taking the turbolift down to the Lab/Archive level, it does not work again.
