Item (Wide-Angle Voth Transphasic Torpedo Launcher) disappeared

I previously submitted a ticket on this (968055), but in their latest response the GM asked me to post a new/separate report over here: I've pasted in below my original submission and the chain of responses since then:

I was changing ships and loadouts at the "Manage Ships" terminal on DS9 when my Wide-Angle Voth Transphasic-Chronitron Torpedo launcher just disappeared. When I mouse over its spot in the loadout it says it is equipped on another ship, but it does not actually appear on any ship, in my bank or inventory, or on the list of items available to be added in the "Manage Ships" menu. If it came from a store ship, I would just discharge my Bulwark ship and reclaim it, but since it was from a Phoenix box I'm afraid I won't be able to reclaim it if I discharge the ship. Please replace this item (or the Bulwark ship, whatever works best).

I have now checked Gornpanther and all 16 of my other characters, and the missing Wide-Angle Voth Transphasic-Chronitron Torpedo Launcher item is still nowhere to be found. I checked all the toons' ships, inventories, bank slots, and shared bank slots, but I'm not sure what you mean by checking the overflow - none of the toons had any overflow windows open. (I'm familiar with overflow windows opening up if I have excess DOFFs or too much loot to fit in my inventory, but I don't know how to check them manually and I have so many open inventory slots on every character I haven't had one of those windows open up in a very long time). Nobody in chat tonight seemed to know how to check overflow manually either - can you tell me what that involves?

It is still showing up (greyed out) on one of Gornpanther's ships that was involved in the starship/loadout switch when the item first disappeared. Mousing over it still generates the message that the item is equipped on a different ship, even though it is not actually equipped on any ship at this point.

I am not sure about the Mk level and quality. I don't believe I had done anything about upgrading it yet, so it was probably Mk XII very rare, or whatever would have come with the ship when it was originally unboxed by my level 65 character.
