Multiple Issues With TFOs (TFO completion fails, ships failing to spawn, etc.)

I've observed a number of issues in Elite TFOs which have made completing TFOs either aggravating at best, impossible at worst. Some of these issues I've observed in advanced TFOs, too, but took longer for me to catch on.
-Herald Sphere, ships during the Destroy Command Ships phase will often either fail to spawn entirely, or stop spawning after two or three ships.
-Herald Sphere, there's an issue at the end of the second phase where some of the portals won't close, and/or the exit portal won't render for some/all of us in the TFO.
-Elite Borg Disconnected (elite, regular), a run I participated in completed prematurely during the transition to the boss fight, marked the boss portion as completed but gave us marks for a failed run. It didn't show us which portion we failed. We had rescued enough Borg ships. The bosses did spawn, though.
-Gateway to Gre'thor (advanced, elite), the TFO will (sometimes?) fail to register the completion of the final phase, Destroy the Herald Forces. This wasn't a big issue during advanced runs, asides from wasting some time, but resulted in a fail during the elite run. (I know we cleared them out during the elite run because I did a search for other ships that may have been missed and found none.)
