Bird Cage TFO: Mission Cannot Progress (2 - Missing Inquisitor)

On one occasion, I have observed the Bird Cage TFO enter a state where the mission cannot progress because the Inquisitor does not appear in Ward C (GenPop) to access the prisoner database. Consequently, it is impossible to proceed to Ward D (Isolation).

Unfortunately, I didn't notice anything else unusual, so I can't guess at a cause.

I have seen this as well. I have looked around a bit in the forums and online and did find some references to similar issues. Today we had an incident where the mission stopped upon entering the main cell block near the start. The destructable consoles were not present when the team arrived.
There was one obnoxious player who pointed to speed running as being the cause. This may be the case as we did progress pretty quickly but this did not seem to be an issue in earlier runs. Not sure if this is the same problem or two different problems but they both result in the mission getting stuck.
