Character has Frankenstein hands

I haven't done anything to my character, but since the latest patches her hands don't look right, her wrists don't match with her arms, it's like they're not joined properly, kind of like they been bolted on like Frankenstein.

Look at the wrists in the image.

Funny you should say. Something I've noticed for a while is that the thumbs don't seem to scale with the rest of the hand, so if you increase the size of your hands they end up looking slightly odd.
