What race are you trying to do this one? I seem to recall that it was possible with a very specific human female face type and complexion combo with I'm not 100% sure and even then it was this specific race and gender and very faint even then.
The first rule of Space Barbie is 'make an alien'. The only exception to that rule is if you want a character that absolutely cannot be made as an alien, like a Gorn. But if you are making anything that can be made as an alien (a human is the most obvious example) you are only limiting yourself by not making an alien.
As far as dimples go, unfortunately I don't see anything that really simulates that look. I checked the various tattoos, complexions, and patterns, and none really look like dimples. The most promising candidate is a tattoo named "Basilisk":
I put it on his forehead and colored it black so it could be easily seen. However, if they allowed you to widen the distance between those spots while keeping their same shape and size (which they don't) you could place them on either side of the mouth and change the color closer the the skin (but still dark enough to stand out) and simulate dimples.
But unfortunately the only way to get them on either side of the mouth involves stretching them with the sliders which completely ruins the look:
As great as STO's editor is, unfortunately it doesn't let you do some really simple things.
But generally speaking, the alien is by far the best species for customization. You can create things that look like Human, Klingon, Andorian, Vulcan, Orion, Cardassian, plus all kinds of really weird things like this Bug Man one of our contributors made. And pretty much all of the costumes in our Featured Index were made using the alien species.

@stocostumes#8412 Thanks for the info and for trying. It occured to me just recently that the only way dimples would really show up would be if we could make the character smile. And except for certain emote actions, like winking and such, we can't.
But I will keep your suggestions in mind and see if I can get some sort of result. If I should achieve any sort of success, I will let you know.
