Missions Giving Avenced TFO Notifications

Been checking back on a recent mission play with one of my STO characters and keep seeing notifications my ship getting these TFO Advanced notifications from Bridge console malfunctions to hull fractures and I've never had this issue and reported to the matainance team. I'm here to submit this issue for anyone who can help out to what's causing this? Has anyone else dealing with this issue? It's only happening to one of my characters.

    What difficulty are you playing at? If you are playing on normal, you do not get any injuries. If you are playing on advanced or elite, you can get injuries. Make sure that your difficulty was not changed from normal to advanced or elite when you played these TFOs.
