Passive skill randomly placed on skill tray

The passive skill, Medical Nanite Generator-Scan, is showing on my skill tray on ground maps. It randomly shows up in a random slot even with the skill tray locked.
When I unlock the skill tray and remove the passive skill and then lock the skill tray, my next map move will result in the passive skill being randomly re-slotted on the skill tray. I have also seen the skill randomly slotted after using another skill. For example: I unlock the tray, I remove the passive skill from the tray and lock the tray, then while fighting enemies I click one of my skills and ~poof~ the passive skill suddenly appears on the tray, while locked.
This is only an issue on one of my toons, Tibs@darpink, none of my other 6 toons have this issue.
