Game TFO crash after crash after crash

Over the last few days STO on Xbox has been crashing constantly when entering a TFO. Sometimes I get in get halfway through the briefing and it crashes. Or I get all the way through the briefing and then it crashes. Makes no difference the map, happens with almost any map at any time. Sometimes I even start the battle and then crash. 90% of the time I end up with a lousy 30 min penalty because of the stupid crash, the other 10% I actually make it back into the TFO and can finish it.
This is starting to occur more and more frequently, only one account signed in on the Xbox (not that that seems to matter as it still crashes, regardless) In one game, one I just played 3 players including myself crashed out of the game TFO was swarm, that left 2 players trying to do what it normally takes 5. By the time I got back in (lucky this time) no ships had been powered up and that meant it was impossible to then repair the satellites and you can't do that when being hit by a 1000 enemy ships.
Finally finished swarm and q'd up again for another, get into the TFO and bam you guessed it Crash city once again, this time Hypermass, which I am glad it crashed on anyway as that TFO is the worst. So for tonight's playing that make 5 crashes within the space of 3 hours .
I even purchased a brand new hard drive in the hopes that might solve the issue and downloaded STO again onto the new drive, but alas no the crashes still happen and now they are happening more and more frequently.
So get rid of the Leaving penalty or fix the game, because it is really starting to annoy me and it is the leaving penalty that is the thing that really annoys me, it's like hey our game crashed so here's a kick in the teeth to go along with it.
