DOFF Assignments Changing to Sick Bay

in PC Gameplay Bug Reports
I'm having about 20% of my DOFF assignments change to Sick Bay and award 63 Medical points. I'm trying to identify which missions or category of missions are changing. So far I can't discern a rhyme or reason. Anyone else having this issue? It's quite annoying.

    Usually, "Sick Bay" doff assignments happend to to a regular doff project 'failing', causing an injury to your officer(s).

    It is possible to critically succeed a mission but still end up in sick bay. Extreme risk assignments have a higher chance of going to sick bay regardless of the outcome. None risk assignments generally don't result in sick bay even if you fail.
