BUG - Cloaks etc. being left behind when characters beam out or disconnect.

So multiple times since the last couple of patches i have seem this happening with other player characters.
When they beam out of a ground map or leave the game and "fade out" or whatever if they have a cloak piece of outfit (Romulan, colony, competitive rep) the cloak pieces are left behind:
I believe this one shown here is part of the Competitive reputation visual set as there was a guy wearing it standing near me for a few mins on this map.
I've seen this happen on DS9, ESD and in the Sompek "after action" map as well. This may be happening when i beam out also as some of my toons have cloaks on, but obviously i can't see the outcome once i leave the map.
