A rather irritating and very intermittent bug I've experienced with Admiralty missions

So here's my deal... on more than one occasion, I've failed admiralty missions that have a 100% chance of success. I understand that even a 99 percent outcome has the potential to fail. But the "Tour of Duty" missions I will NOT do unless I have that 100 percent chance of success. Yet...I failed several. So...how do I fail something that in writing, has a ZERO percent chance of failure?
