"Mindscape" is a Buggy Mess

"Mindscape", mission two of the Delta Quadrant Arc, is an absolute mess. Graphical glitches, advancement glitches, and all sorts are abound in this mission. I'm going to list all the bugs I identified below. I hope these issues get resolved as a result - playing on my Agents of Yesterday character, these bugs are preventing me from being able to advance in the Temporal Agent milestones.

Bug 1: When first entering the mindscape, in the sickbay, the computer panel mounted on the wall flickers due to the model being partially embedded in the wall, making for a distracting and unappealing side object.

Bug 2: If the player character dies at any point during the mindscape section, your character is respawned on an empty version of Voyager's bridge, completely unable to progress through the mission, instead having to beam up and then restart.

Bug 3: After defeating Cooper on the bridge of the Voyager in the mindscape, before the second iteration, there is a chance upon transitioning to the second iteration that Tuvok - as a squadmate - will still spawn. This will make you unable to talk with Tuvok - as an NPC - until the squadmate despawns after an unspecified amount of time for a reason I can't figure out.

Bug 4: When playing the tricky hallways section - the one leading to Astrometrics - for the last section, during the opening of Astrometrics, Tuvok loses all sense of pathfinding and stays completely still in a different corridor, thus meaning he does not assist the player during the fight.

Bug 5: Upon completing the final objective in "Mindscape", instead of giving the option to hand in the mission to the Delta Quadrant mission contact, the mission objectives completely restart - you are unable to hand in the mission as completed, and instead, the mission resets to the beginning, forcing you to start it again. Having played through it in this way, I can confirm that it is an infinite loop, and does not magically fix itself after the second time.

These bugs have meant, over the last few days, I've replayed the entirety of the mission four times, but have never been able to hand it in as completed due to the last bug, and the others impeded progress significantly. This is a dire error and should be resolved ASAP.

    Most of those are still there in 2020... (Yes Necro I know), with a lovely new shiny one of "almost all the attacking undine on the bioship are invisible".
