"Shattered" title missing

Completed the "Crystalline Cataclysm" PvE mission yesterday (event version), got 1st place (dropped 2 random very rare space gear as part of the reward), got the "Shattered" accolade, it *said* I got the "Title: Shattered", but... no title. Got the same on another character last week, and that character got the title. Managed to get 1st place on the same character today (the one with the accolade but not the title), thought it might recognize the missing title and award it, but... nothing. While it doesn't affect game play, not awarding the title seems to defeat the purpose of telling the player it was awarded.
Note: I don't know whether the event makes a difference; the other character that got it did so while the event was "officially" running - this character hit it twice during the "extension". I don't know if it's possible to obtain outside the event during the "Crystalline Catastrophe" (non-event) version of the mission. According to this:

however, checking under General Accolades, the accolade name/description appears to be the same for each character. Regardless, even if there's supposed to be two (which should be described accordingly if so), since the event was "extended" and 1st place was obtained during the "Crystalline Cataclysm" (event) version, it should have awarded the corresponding title (as it stated on screen during the mission at the time). One way or another, this needs to be fixed.
