Titles & Passives from Accolades [PS4]

The following is taken from a Support Ticket I wrote to Arc. They suggested that it was a gameplay bug and that I post it here for the development team to see.
The top tier Accolades which award 50 points, a title and a passive trait, do not seem to be functioning.
Although the title becomes unlocked, the passive traits seemingly do not function, nor do they appear on the Captain's passive skills list under their stats.
I have done some testing. For example: When equipping the Executioner Title, the bonus 2% Plasma damage for ground Plasma weapons does not apply, and the weapon's damage stays stationary.
This may in fact be my misunderstanding, and the increase takes effect regardless of whether or not the Title is equipped. However, the one thing that does seem to be an issue in this case is that none of the passive bonuses awarded by completing tier 3 Accolades are appearing on the Captain's list of passives, which leads me to believe they are currently not having an effect.
