    I have seen that (researching....) it appears that your transporter operator either needs a LOT more training, or could even be an Undine in disguise attempting to disable your ship by killing off your crew. When a transport goes awry and attempts to materialize something into a spot already occupied by something else, or tries to transport to separate items into the exact same spot....well, the results aren't pretty

    Reminds me of a particular TNG episode where part of the floor inside the ship dematialized underneath a female crewman and materialized again cutting them in half and instantly killing them.

    Going on for years and still no fix in sight. Take a look at this from May 2016, taco gave a bit of an answer as to why this happens, but unfortunately also that it'll bee a long time until someone will fix it.
