I think it matters what the danger level of the mission is... if it's 'extreme' and you have any chance of mission failure, there is a chance you'll lose a doff.
Green or better will only go to sick bay. Whites will take a dirt nap on disaster.

They can also die on a success (dunno for certain whether on a crit, too, but I wouldn't rule it out), especially on the higher danger levels. But yeah, it is kind of a bummer that those R&D doffs on AoY toons are commons.
Also, I had a doctor die on a common healing mission (the "fractured arm" type).

These doffing assignments do give a warning in the description that this will happen though ("officers assigned to this will be lost permanently" in red)

Greens are good for the grinder or officer exchange fodder, unless youre new and really starting out theyre a bit TRIBBLE all around. Whites you should just assume sell them and then buy a green of the same specialty if youre in that low level new.player boat.. the white to green price difference for most specialties is pretty negligable.

I've had common DOFFs die by failing in playing Dabo. KDF rolls hard like that.
