Having the same issue, trying to find a correlation to when its happening. All toons in any ship have this issue and it seems to be happening when I go to fire weapons after Buffing up a bunch of tactical BO powers, but still researching this.
This happens a lot when I cause enemy ships to explode. I can also replicate this 100% whenever I use the phaser lance on my Phantom Intel Escort. Hope this helps.
EDIT: I was able to replicate it consistently with my phaser lance, now suddenly tonight I can't. So it's just completely random as far as I can tell. My ship will just de-spawn, and re-spawn in the middle of combat, and if I turn my camera around to face behind me and I watch my ship warp back in from the distance.
I have it from multiple solid sources that PC bugs and legacy bugs will be lower priority for the near future simply because of the console releases. So smoke 'em if you got them.
Basically PC got F'ed so bad now because our console counterparts have a huge list building up..I'll get the beer cause this is gonna be a long wait and I think all the beer in the world will be over before they fix the PC list..

This bug is already so long and the are doing nothing about it, or let us know what the problem is that the can not fix it. I have any reason to say about it, because I am a life time member and also pay for the game. I know there are more people that also pay for the game and have the reason to be heard of. So the only thing I can say, just contact the admin directly and make a support ticket. Also talk to them to the support channel in the game and let them hear us. Because we don't earn to be ignored by them if we also pay for the game.
I wonder if this is also related to the problems with the "Rock and Roll" Pilot ability. No animation, partial/halted animation, randomly delayed/doubled animation.

^^ I wasn't kidding. Especially in things like the Terran battlezone, where you're fighting one of the 3 stations, and you constantly do a full warp-out, causing you to have to fly back up there at full throttle. Totally kills your DPS, and effectively makes the map unplayable.
Seems ability related, and mostly Science I think.
I am still having this problem and perfect world is doing nothing about it and at is almost a haf year now!!! There are members paying for the game as I do, but the only thing the can do is band me from the forum. I only say the truth and the can not handel that.

I too have the same problem , during the battle while active overload the ship goes into warp disappears and reappears .
