Voice acted lines keeping breaking down randomly during some patches.

This is becoming bothersome. The more content is added to the game, the more older voice-acted lines are broken in a way or another.
Among many other examples:
-Van'Zyl's VA don't match her lines at the beginning of Uneasy Allies, this one has been present for a several months now
-Tasha Yar and Castillo stop talking once you reach the corridor leading to the Enterprise-C in Temporal Ambassador
-a line of Torg from The House Always Wins is repeated twice
-Most of Borg Disconnected VAs are gone
-Half of Mirror Incursion Event VA are gone
And before they were fixed, there were:
-Kahless from House Pegh missing various VA
-a line from Taris when Shon comes to the rescue of Qo'Nos in Surface Tension
Most of them apparently unrelated to the content that was released with the patches.
I suggest the audio team to check if there isn't a bigger problem at hand for the voice-acted lines to avoid this kind of missing voices in the future.
