When upgrading equipment, sometimes I have found that when you fill the technology point bar and finally try to upgrade the item, there are times where the item is available for claiming immediately, and the item is not even upgraded even though millions of EC and thousands of Dilithium were spent.
This happened to me on two of my consoles which I wanted to upgrade from very rare to ultra rare quality (both consoles mk XIV) and again to a dual phaser beam bank (mk XIV) from very rare to ultra rare.
What is going on? I'm very aggravated to find that all of my efforts gathering currency to upgrade items is simply lost on a failed upgrade.
I submitted a ticket in game, and have yet to hear anything back for over a week now.
Is this happening to anybody else?

As it is very clearly shown in the upgrade window increasing the quality is chance based. If you don't have a chance of 100% you can fail and the items stays the same.
As mentioned, its not a bug. There are two bars on the Upgrade UI, the top bar is Technology Points, and the bottom bar is Chance of Quality Improvement on Upgrade.
When you fill the Technology Points bar you can upgrade the Mark of the weapon, up to Mark XIV which is the current maximum.
When you spend points it also will gradually increase the Chance of Quality Improvement on Upgrade bar. If you get lucky while trying to upgrade to Mark XIV and crit, your item will increase in quality, you will see this happen as soon as you click the Apply Upgrade button, the output will show an improved quality. If you don't get luck, your crit gets set back to zero when you upgrade.
After you hit Mark XIV and fill the Technology Points bar, if you don't get a crit when you hit Apply Upgrade, it will just give you the item back, however, any crit chance you earned you will keep until it crits into a higher quality item.
