I've noticed disappearing DOff missions too. It's very noticable because I have 3 characters that I reset the two Dilithium Mine Doff missions on every evening. At first I thought I was just being an idiot and getting distracted before setting the missions up properly, but now there's no doubt about it: missions definitely sometimes disappear on 'completion'.

To note, I don't think it's just those two missions, but because they're dailies and I'm specifically looking for them to claim so that I can contribute the mine resources (if any) to fleet projects, I notice they're missing.

I can even watch a character on login go from, to give one example from a few minutes ago, 7 missions In Progress, to 0 In Progress, 6 Completed.

I can't 100% reproduce it every time, but this is definitely not a one-off event, or tied to a specific character.

The weird thing is the point at which it's failing. For example, I had a character lose the 4/4 mission for one of the diplomatic chains on complete, and it still credited in the 'Mission Chains' tab for having completed it even though I wasn't able to claim the completion bonuses for it. It also must not simply be hanging them out there somewhere unclaimed but invisible, otherwise I wouldn't be able to begin the Dil Mine Doff missions again.

Doff assignment chains are like that. Most of them you only have to complete the assignment in the chain. It doesn't have to be successful to count. There are a few exceptions such as an assignment in a chain awarding a special doff or special item upon successful completion; if it's failed you might have to try again after finding the assignment again.

Seems any DOff assignment can disappear randomly. I had 17 In Progress which changed to 0 In Progress, 14 Complete. None of disappeared wasn't for assignment chain or any other way important.
Seems if having more than 5 assignments in progress, usually at least 1 disappears.

I just had this happen to me. I had switched to 1 character then back. I was on the Completed tab and it looked like an assignment was finishing but instead of a completed assignment showing up in the Completed tab it just suddenly said I had 1 available assignment slot after a few seconds of finishing. I think it was the Confiscated Contraband assignment but it does seem to be random assignments and happens on multiple characters. Most or all of my characters each day are showing less assignments in progress and completed than I actually started. I make sure to fill all assignment slots on each character each day. For example: I could start 23 assignments on 1 character and the next day it shows I have 18 finished and 3 or 4 in progress for a total of 21 or 22 assignments. On another character I can have 18 assignments finished with none in progress and 2 available slots.

So somehow I'm losing 1 or 2 assignments on most or all characters each day.

Another thing I noticed is when logging in on a character for the first time in a day I have a Sick Bay assignment stuck on finishing. Sick Bay assignments won't finish at all until you claim the rewards/acknowledge failure of 1 completed assignment. Note: Sick Bay assignments don't take up assignment slots so you could have 1 or more in progress and still be able to start 20+ assignments (depending on if you bought any extra slots from your fleet embassy).

From the looks of things my characters with disappearing assignments seem to always have at least 1 Sick Bay assignment where doffs get injured due to the assignment completing as a disaster. I also noticed I seem to be getting doffs sent to Sick Bay a lot more across all characters lately.

I also noticed that assignments complete instantly now. No more waiting for them to complete 7 at a time. Something must have gone wrong when they fixed the slow finishing issue recently.

I've noticed the same with the Dil Mine missions lately. I ran them on 11 characters yesterday and two had disappeared when I logged in to claim them today. I think I saw that Cryptic is aware and investigating.

Theory: assignments that give you nothing at all when they fail or end in a disaster are being deleted instead of going into your Completed tab and having you Acknowledge Report. (The reason why I think it's only assignments that award nothing is because I have been seeing failed 'Haggle for [commodity]' assignments in the completed tab and those types still give you at least a couple items even if it fails, though they give you nothing if they end in a disaster.)

I noticed on 1 character missing an assignment he didn't have any Sick Bay assignments in progress or completed. On the next character I had 1 assignment missing, 1 Sick Bay assignment completed, and noticed that not a single completed assignment was a failure or a disaster. They all showed up as successful.

Having a Sick Bay assignment with no failed assignments at all in the completed tab clued me in on this theory. There should have been 1 assignment that ended in a disaster, which is the reason why my doff wound up in Sick Bay, but the failed assignment was nowhere to be found.

Edit: I had another character missing a couple assignments and none of the assignments in the Completed tab ended in a failure or disaster. 1 Sick Bay assignment with 1 doff meant that at least 1 of the missing assignments ended in a disaster, the other was probably just a failure that didn't put doffs in sick bay. Mystery solved?
