Failing the non-optional objectives in Khitomer in Stasis Advanced (also known as Khitomer Accord Ground Advanced) and leaving results in an unfair one-hour ban from queues. Also reported in these threads which were not filed under Gameplay Bug Reports:
We've failed the mission. People have left before me making it practically impossible to progress through the failed mission to try and avoid the penalty. So, i leave the failed mission and acrue the leaver penalty for an hour when a cd would only be 30 minutes. You would think that this would be something they would want to fix sooner rather than later but its been this way for months. Not to mention the "optional" objective makes absolutely no sense because if you succeed in the mission you are of course going to get the "optional". This lack of logic and incredible amount of bugs drives players away. It has definitely brought me to the verge.
Your last piece of advice in that thread is false, because you still do suffer the leavers penalty. So storms you don't, most most often you do. This issue has been in Khittomer in Stasis forever but no dev has even given it a "gotcha" yet.
maybe you could explain 4th off to me. what does "follow both of those to a T" mean?
For me getting a leaver penalty is a think that sometines happens and sometimes does not.
If you have failed the missions optional and there fore the mission you already have been in the mission for 15min because thats the time yuou have to destroy the IMRs. Even if you go ahead and kill the remaining Borg inkl. Tosk you don`t geta a beam out Button, no matter how long you wait. But sometines you get a penalty, dsometinmes not. seems that even that is broken.
Aside from that, and i don't want to start another discussion about optionals or so): As they changed the difficulty of those Missions with DR they mixed optional and objective up. The IMRs were and are supposed to be optional and the other part with Tosk was mandatory.
Everyone who is not completely blind can see that its useless to have more time for the optional than for the mandatory.
If you blow up the IMRs within 15 min you also won the optional even if it says "complete the mission in 20+ minutes (don't remember the exact time)". So the optional should be tied to Tosk (complete the Mission) but its tied to the IMRs.
And if you fail to destroy them you can't complete the optional even if the Timer is still running so that got mixed up and i think that is also what broke the mission.
A simple change to make the IMRs optional again (with Timer) and Tosk mandatory (no Timer) would be enough and should fix everything
Ongoing problem since they changed a lot of the old optional objectives to be mandatory, but for some reason never updated the leavers penalty info to account for the new fail conditions on certain missions. It was reported during testing and during the initial release but no devs seem to have actually done anything about it. Then they wonder why so many queues are dead.

If you fail the mission in your example, than it is because you failed to destroy the IMR's in time, however the time do destroy them vs the time to complete the mission, are 2 different entities.
Therefore, if you do not destroy the IMR's in time, than also by your example, you must wait the full 15mins. of mission completion time, before beaming out due to not offering an automatic leave mission button option [including timer for auto-booting].
I have tested this on thousands of occasions and, it has always worked provided what I described is followed to exactness.
Leaving a STF that fails, with no automatic leave option before a full 15mins. of actual in mission time has expire, will almost certainly result in a leavers penalty.
Waiting for the full 15mins. or, greater amount of time to have expired, will not net a leavers penalty even if you are the first to leave.
If the mission fails and, an automatic leave mission option appears with a timer [you must click this and, not the manual beam out and, you will not incur a leavers penalty.
