Ship equipment resets to default (KDF)

I am creating a new char (sci / orion)
at a few missons where i warped in, it would reset the equipment to the default of what the ship comes with when you buy it.
And i have had a few times that the equipment i had om my ship, deflector, shield, engine and a DHC disruptor that i changed out from the default ones, got changed back to the defaut ones, the (better) ones that i equipd where back in my inventory (i kept the default ones in my inventory).
This happend at least 2 times on the empire serie's missons (i was using a b'roth BOP)
And also on the warzone - bring down the house (somraw raptor)
It only seems to reset the slots. but ones that where empty are not affected.(console slots)
Easy fix right now is to save the loadout and just load it if it happends. but still, should not be:D
